- TRINITY HEALTH PACE ALEXANDRIA IS A PROGRAM OF ALL-INCLUSIVE CARE FOR THE ELDERLY (PACE), SERVING SENIORS IN RAPIDES AND AVOYELLES PARISHES IN LOUISIANA. OUR UNIQUE PROGRAM PROVIDES FULL-CIRCLE CARE TO SENIORS SO THEY CAN REMAIN INDEPENDENT AND AGE SAFELY AND COMFORTABLY IN THEIR OWN HOMES.TRINITY HEALTH PACE ALEXANDRIA OFFERS A FULL SPECTRUM OF HEALTH CARE SERVICES TO THE ELDERLY, FROM PRIMARY TO ACUTE TO LONG-TERM CARE. SENIORS WHO QUALIFY FOR NURSING HOME CARE ALTERNATIVELY HAVE THE OPTION TO RECEIVE COMPREHENSIVE CARE FROM AN INTERDISCIPLINARY TEAM OF EXPERTS, ALL FOCUSED ON IMPROVING PARTICIPANTS' HEALTH.
