KERALTY COMPASSIONATE COMMUNITIES (KCC) IS DEDICATED TO IMPROVING HEALTH AND WELL-BEING BY ADDRESSING SOCIAL DETERMINANTS OF HEALTH (SDOH). KCC BELIEVES THAT TRUE HEALTH GOES BEYOND MEDICAL CARE - IT IS SHAPED BY THE CONNECTIONS, ENVIRONMENTS, (CONTINUED ON SCHEDULE O)

990 for the year ended December 31, 2024

Program accomplishments

  • KCC'S PROGRAM ACTIVITIES ARE CENTERED AROUND IMPROVING THE OVERALL QUALITY OF LIFE FOR THE COMMUNITIES IT SERVES BY ADDRESSING SOCIAL DETERMINANTS OF HEALTH (SDOH). KCC'S COMMUNITY OUTREACH IN 2024 INCLUDED SIX WORKSHOPS ON THE TOPIC OF MINDFULNESS CONDUCTED FOR HISPANIC UNITY OF FLORIDA, A NONPROFIT ORGANIZATION WHOSE MISSION IS TO EMPOWER IMMIGRANTS AND OTHERS TO BECOME SELF-SUFFICIENT AND PRODUCTIVE. DURING 2024, KCC HOSTED THE FIRST COMPASSIONUNITED US VIRTUAL SUMMIT, AN EVENT THAT BROUGHT TOGETHER LEADERS, CHANGEMAKERS, AND COMMUNITY ADVOCATES TO CELEBRATE THE POWER OF COMPASSION AND EXPLORE ITS ROLE IN CREATING MEANINGFUL CHANGE WORLDWIDE THROUGH COLLABORATION AND CONNECTION ACROSS DIVERSE COMMUNITIES. KCC ALSO PARTICIPATED IN A SUMMIT HOSTED BY THE COALITION TO TRANSFORM (CONTINUED ON SCH. O)ADVANCED CARE IN 2024, PROVIDING OPPORTUNITIES TO SHARE BEST PRACTICES FOR ENHANCING CARE FOR THOSE WITH SEVERE ILLNESSES AND TACKLING THE CHALLENGES OF SOCIAL ISOLATION. KCC FURTHER ENHANCED ITS MISSION IN 2024 THROUGH THE INTRODUCTION OF THE CAREGIVER GUIDE, A RESOURCE DESIGNED TO PROVIDE ASSISTANCE FOR CAREGIVERS NAVIGATING END-OF-LIFE CARE. KCC'S ONGOING INITIATIVES TO BUILD SUPPORTIVE AND CONNECTED COMMUNITIES ARE FOCUSED AROUND THREE MAIN PRINCIPLES:1. COMPASSION AND HUMANIZATION- HUMANIZING WORK TO REDUCE BURNOUT AND STRENGTHEN CARE:BURNOUT AFFECTS PROFESSIONALS ACROSS INDUSTRIES - HEALTHCARE WORKERS, EDUCATORS, SOCIAL WORKERS, FIRST RESPONDERS, AND MANY OTHERS WHO DEDICATE THEMSELVES TO SERVING OTHERS. THE EMOTIONAL WEIGHT OF CARING FOR PEOPLE, ENGAGING WITH THEIR STRUGGLES, AND RESPONDING TO HIGH-STRESS SITUATIONS CAN LEAD TO PROGRESSIVE DISCONNECTION FROM LIFE'S PURPOSE, PSYCHOLOGICAL STRAIN, AND EMOTIONAL EXHAUSTION.KCC DEVELOPS HUMANIZATION STRATEGIES THAT ADDRESS BURNOUT, IMPROVE WORKPLACE WELL-BEING, AND ENHANCE THE QUALITY OF CARE AND SERVICE. KCC'S COMPASSIONATE MINDFULNESS TRAINING PROGRAM EQUIPS PROFESSIONALS WITH TOOLS TO RECONNECT WITH THEIR PURPOSE, BUILD RESILIENCE, AND FOSTER DEEPER HUMAN CONNECTIONS - HELPING THEM CARE FOR OTHERS WHILE ALSO CARING FOR THEMSELVES.2. COMMUNITY APPROACH IN HEALTHCARE - ADDRESSING LONELINESS AND SOCIAL NEEDS IN HEALTHCARE:KCC RECOGNIZES THE PROFOUND IMPACT THAT LONELINESS AND SOCIAL NEEDS HAVE ON HEALTH OUTCOMES. THAT'S WHY KCC INTEGRATES ASSESSMENTS OF SOCIAL CONNECTION INTO HEALTHCARE, ENSURING A MORE HOLISTIC APPROACH TO CARE.KCC'S STRATEGIES HELP THE HEALTHCARE SECTOR GO BEYOND TREATMENT - CONNECTING WITH THE TRUE NEEDS OF PATIENTS, PROVIDING MEANINGFUL SUPPORT, AND FOSTERING STRONGER COMMUNITIES. BY ADDRESSING THESE SOCIAL FACTORS, KCC IMPROVES OVERALL WELL-BEING AND CREATES A HEALTHCARE SYSTEM THAT TRULY CARES FOR PEOPLE AND THEIR COMMUNITIES.3. COMPASSIONATE COMMUNITIES - EMPOWERING COMMUNITIES TO ADDRESS SOCIAL DETERMINANTS OF HEALTH (SDOH):THE NEED FOR INNOVATIVE SOLUTIONS TO ADDRESS SDOH IS MORE URGENT THAN EVER. KCC BELIEVES THAT THE MOST EFFECTIVE SOLUTIONS COME FROM WITHIN COMMUNITIES THEMSELVES.KCC'S BOTTOM-UP APPROACH PRIORITIZES COMMUNITY VOICES AND LIVED EXPERIENCES. BY ACTIVELY LISTENING TO RESIDENTS, KCC SUPPORTS THEM IN IDENTIFYING RESOURCES, DEVELOPING THEIR OWN STRATEGIES, AND DRIVING MEANINGFUL CHANGE. THROUGH THESE EFFORTS, KCC HELPS TO LOWER NEGATIVE HEALTH OUTCOMES LINKED TO SDOH AND CONNECT COMMUNITIES WITH KEY STAKEHOLDERS WHO CAN AMPLIFY THEIR IMPACT.BY EMPOWERING PEOPLE TO SHAPE THEIR OWN FUTURE, KCC CREATES STRONGER, HEALTHIER, AND MORE RESILIENT COMMUNITIES.
