Program accomplishments

  • RESIDENTIAL PROGRAMS MERAKEY PHILADELPHIA'S ADULT BEHAVIORAL HEALTH RESIDENTIAL PROGRAMS CONTINUE TO SERVE AS A CRITICAL COMPONENT OF THE CITY'S BEHAVIORAL HEALTH CONTINUUM FOR ADULTS WITH SEVERE AND PERSISTENT MENTAL ILLNESS. SPANNING MORE THAN 122 RESIDENTIAL BEDS ACROSS LONG-TERM STRUCTURED RESIDENCES (LTSR) AND COMMUNITY RESIDENTIAL REHABILITATION (CRR) PROGRAMS, THESE SERVICES PROVIDE CLINICALLY INFORMED, RECOVERY-ORIENTED HOUSING DESIGNED TO PROMOTE STABILITY AND LONG-TERM SUCCESS IN THE COMMUNITY.IN CLOSE COLLABORATION WITH THE PHILADELPHIA DEPARTMENT OF BEHAVIORAL HEALTH AND INTELLECTUAL DISABILITY SERVICES (DBHIDS), THE PROGRAMS MAINTAIN AN EFFICIENT AND COORDINATED REFERRAL PROCESS THAT ENSURES TIMELY ACCESS TO APPROPRIATE LEVELS OF CARE. THIS PARTNERSHIP SUPPORTS CONTINUITY, MINIMIZES SERVICE GAPS, AND STRENGTHENS OUTCOMES FOR INDIVIDUALS TRANSITIONING FROM HIGHER LEVELS OF CARE.IN THE PAST YEAR, THE RESIDENTIAL PORTFOLIO DEMONSTRATED EXCEPTIONAL REGULATORY AND OPERATIONAL PERFORMANCE, INCLUDING A SEAMLESS OMHSAS STATE AUDIT COMPLETED WITH ZERO CITATIONS. THIS ACHIEVEMENT REFLECTS STRONG LEADERSHIP, CONSISTENT ADHERENCE TO REGULATORY STANDARDS, AND THE DEDICATION OF MULTIDISCIPLINARY TEAMS WHO PRIORITIZE QUALITY, SAFETY, AND PERSON-CENTERED CARE.ADDITIONALLY, THE PROGRAMS HAVE SUSTAINED A THERAPEUTIC RESIDENTIAL ENVIRONMENT THAT SUPPORTS RECOVERY, REDUCES UNNECESSARY HOSPITAL UTILIZATION, AND FOSTERS INCREASED INDEPENDENCE FOR RESIDENTS. COLLECTIVELY, THESE ACCOMPLISHMENTS UNDERSCORE MERAKEY'S CONTINUED COMMITMENT TO DELIVERING HIGH-QUALITY RESIDENTIAL SERVICES THAT PROMOTE DIGNITY, STABILITY, AND MEANINGFUL PROGRESS TOWARD RECOVERY.COMMUNITY INTEGRATED RECOVERY CENTERS (CIRCS) MERAKEY PHILADELPHIA OPERATES TWO COMMUNITY INTEGRATED RECOVERY CENTERS (CIRCS), COLLECTIVELY SERVING OVER 150 MEN AND WOMEN WITH SEVERE AND PERSISTENT MENTAL ILLNESS. THESE PROGRAMS ARE DISTINGUISHED BY THEIR INNOVATIVE USE OF CREATIVE ARTS THERAPIES INCLUDING MUSIC, VISUAL ARTS, AND MANUAL ARTS TO SUPPORT CONSUMERS' RECOVERY JOURNEYS. BY FOSTERING SELF-EXPRESSION AND EMOTIONAL HEALING, THESE THERAPIES ENHANCE PARTICIPANTS' OVERALL WELL-BEING AND ENGAGEMENT IN THEIR RECOVERY PROCESS.OUTPATIENT MENTAL HEALTH CLINICS MERAKEY PHILADELPHIA'S OUTPATIENT MENTAL HEALTH CLINICS PROVIDE CRITICAL SUPPORT TO A DIVERSE POPULATION OF OVER 2,300 ADULTS AND CHILDREN. THESE CLINICS DELIVER TARGETED, EVIDENCE-BASED TREATMENTS DESIGNED TO ACHIEVE MEASURABLE IMPROVEMENTS IN MENTAL HEALTH OUTCOMES. BY EMPHASIZING SHORTER, GOAL-DIRECTED EPISODES OF CARE, THE CLINICS EMPOWER INDIVIDUALS TO ACHIEVE LASTING PROGRESS WHILE OPTIMIZING SERVICE EFFICIENCY.COMMUNITY-BASED PROGRAMS MERAKEY PHILADELPHIA'S COMMUNITY-BASED PROGRAMS UNDERWENT A STRATEGIC CONSOLIDATION IN FY24 TO ENHANCE COLLABORATION AND SERVICE INTEGRATION. BY BRINGING TOGETHER TRADITIONAL CASE MANAGEMENT, MOBILE PSYCHIATRIC REHABILITATION, AND PEER SUPPORT SERVICES, THE PROGRAMS NOW OFFER A MORE COHESIVE AND HOLISTIC APPROACH TO MEETING CONSUMERS' NEEDS. THIS STREAMLINED MODEL FOSTERS STRONGER CONNECTIONS BETWEEN SERVICES, IMPROVING ACCESSIBILITY AND OUTCOMES FOR INDIVIDUALS RECEIVING CARE.INTENSIVE BEHAVIORAL HEALTH SERVICES (IBHS) SCHOOL PROGRAMS MERAKEY PHILADELPHIA'S IBHS SCHOOL PROGRAMS PLAYED A PIVOTAL ROLE IN HELPING STUDENTS REACCLIMATE TO CLASSROOMS FOLLOWING THE CHALLENGES OF THE COVID-19 PANDEMIC. THESE PROGRAMS PROVIDED TARGETED BEHAVIORAL AND EMOTIONAL SUPPORT TO ADDRESS THE UNIQUE STRESSORS STUDENTS FACED DURING THE TRANSITION BACK TO IN-PERSON LEARNING. INITIATIVES INCLUDED SOCIAL-EMOTIONAL LEARNING WORKSHOPS, INDIVIDUALIZED SUPPORT PLANS, AND PROACTIVE COLLABORATION WITH SCHOOL STAFF TO CREATE TRAUMA-SENSITIVE ENVIRONMENTS. THESE EFFORTS FOSTERED RESILIENCE, IMPROVED CLASSROOM ENGAGEMENT, AND SUPPORTED STUDENTS IN REBUILDING SOCIAL CONNECTIONS CRITICAL TO THEIR ACADEMIC AND PERSONAL SUCCESS.
