Program accomplishments
- Through funding from the Florida Department of Health, Office of Minority Health, Center for Change delivered comprehensive outreach, education, and screening services as part of the Chronic Disease Education and Screening Program. More than 1,500 individuals across targeted communities in Orange, Seminole and Osceola counties received health education focused on chronic disease prevention and management, specifically Colon and Prostate Cancer, Diabetes, and Cardiovascular Health. Center for Change facilitated 10 evidence-based Chronic Disease Self-Management Program (CDSMP), Diabetes Self-Management Program (DSMP), Chronic Pain Self-Management Program (CPSMP) workshops and conducted 4 CDSMP Leader/Train-the-Trainer sessions. These evidence-based programs developed by Stanford University, are shown to improve participants physical health, psychosocial well-being, and overall quality of life while managing chronic conditions. The agency also supported more than 100 individuals through the Healthy Heart Blood Pressure Program, over 60 individuals in the Health Coaching program, and screened more than 225 adult men and women using the immunochemical fecal occult blood (IFIT) test, prostate-specific antigen (PSA) test, and A1C self-check test.Expenses $292,967 · revenue $320,413
- Center for Change partners are an essential part of its most relevant grants with offering a blood pressure monitoring program. One partner ranking as the second most relevant as measured by the revenue and expenses for the calendar year is the partnership with the Winter Park Health Foundation. This partnership started in 2024 and was enhanced and continued for three additional quarters in 2025. During 2025, staff made strategic adjustments to how the program was introduced to the clients which included revised educational materials and a clearer explanation of the programs structure. As a result of these enhancements, Center for Change completed the 12 month program with an enrollment of 111 participants in the Heart Health Blood Pressure Program which exceeded the goald of 100 participants. Of those enrolled, 88 participants successfully completed the program, and 80 of them (approximately 72%) showed improvements in their blood pressure readings. Understanding that our partner (WPHF) has a targeted demographically area to include the cities of Winter Park, Maitland, and Eatonville; the program reached 83 participants or approximately 75% of total enrollees who resided in these targeted areas with an average participant age of 65. This success strongly aligned with the WPHFs mission to promote the health and well-being of these communities.Expenses $38,344 · revenue $25,000
- Through funding from the Colon Cancer Coalition, Center for Change delivered comprehensive colon cancer outreach and education across Orange, Osceola, and Seminole counties. Using Colon Cancer Coalition materials and culturally relevant engagement strategies, the team educated 1,500 adults on colon cancer risks, symptoms, prevention, and the importance of early detection. Education sessions were held at community events, churches, centers, and the Center for Change office, and included surveys to assess participants knowledge and health behaviors. A key component of the grant involved distributing and supporting the use of the immunochemical fecal occult test (IFIT). Seventy-six high risk or screening interested individuals completed the test. The team also amplified awareness through participation in major Colon Cancer Coalition-aligned events, including Get Your Rear in Gear 2025, the Come Out with Pride Festival, the Orange County Government Wellness Expo, and Cure Bowl event. Public outreach also included several informational-educational videos about colon cancer myths and statistics, risk factors, and the benefits of screening placed on our social media platforms.Expenses $3,975 · revenue $15,000
