THE MISSION OF HELEN PORTER REHABILITATION AND NURSING IS TO IMPROVE THE HEALTH OF OUR COMMUNITY, ONE PERSON AT A TIME.

THE FACILITY IS THE ONLY SKILLED NURSING FACILITY IN ADDISON COUNTY, VERMONT.
