R0000-194: Duplicate of a submission already accepted
Form 990-EZForm 990Reject and Stop
Duplicate Condition. Rule text from IRS MeF Business Rules, Form 990-EZ, TY2026v3.0; IRS MeF Business Rules, Form 990, TY2026v3.0. Last reviewed .
What this rejection means
The IRS received a submission that duplicates one it had already accepted. It rejected the copy.
IRS business rule R0000-194: Submission must not be a duplicate of a previously accepted submission.
The severity is "Reject and Stop." The IRS stopped processing this submission at this error and did not go on to check the rest of it.
For you, the key point is in the rule text. An earlier submission was already accepted. This rejection does not undo that acceptance.
Why it happens
This rule concerns the electronic submission package that your e-file software sends to the IRS. It is not about the numbers on your return. Realistic causes include:
- Your e-file software sent the same submission twice, for example after a timeout or a retry.
- Someone pressed "file" again after the first submission had been accepted, because the acceptance notice was missed or delayed.
You cannot see or change the submission package yourself. Building and transmitting it is your e-file provider's job.
How to fix it
- Do not send the same return again. That will not change the result.
- Ask your e-file provider for the acknowledgment of the first submission. If it shows "accepted," your return for that year is filed.
- Check the accepted return's contents. Make sure the version the IRS accepted is the one you meant to file.
- If you need to change the accepted return, file an amended return. The Form 990-EZ instructions say to file a complete return for that year, check the "Amended return" box in Item B, and explain the changes on Schedule O.
- If you have no record of an accepted submission, ask your provider to look into it. The duplicate check means the IRS has one, so the provider should be able to trace it.
When to contact the IRS
You normally do not need to. This is a transmission issue. IRS Publication 4164 names the IRS e-help desk as the first point of contact for e-file issues, including rejects, for transmitters and software developers. If your provider cannot explain the rejection, they are the ones to raise it with the IRS.

Questions
We got R0000-194. Is our return filed?
The rule says this submission duplicates one the IRS already accepted. Ask your e-file provider for the acceptance acknowledgment of the earlier submission; that acknowledgment is your proof of filing, not this rejection.
Do we need to do anything?
If the earlier submission was accepted and correct, usually no. If you need to change what was filed, you file an amended return with the Amended return box checked, not the same submission again.
