Mission
OUR MISSION IS TO ASSIST YOUNG ADULTS IN SUCCESSFULLY TRANSITIONING OUT OF FOSTER CARE BY PROVIDING A CARING NETWORK OF CONSISTENT SUPPORT, CONNECTION TO EMERGENCY SERVICES, AND GROWING LIFE SKILLS.
Program accomplishments
- STRUCTURED MENTORSHIP PROGRAM - FPC HOPE CENTER MATCHED EACH PARTICIPANT WITH A TRAINED ADULT MENTOR WHO PROVIDED CONSISTENT GUIDANCE, ACCOUNTABILITY, AND EMOTIONAL SUPPORT. MENTORS HELPED YOUTH SET EDUCATIONAL AND CAREER GOALS, NAVIGATE LIFE TRANSITIONS, AND BUILD TRUSTED RELATIONSHIPS THAT MANY HAVE LACKED THROUGHOUT CHILDHOOD.
- IMPLEMENTATION OF TRAUMA-INFORMED CASE MANAGEMENT SERVICES - FPC HOPE CENTER EXPANDED ITS SERVICE MODEL BY INTRODUCING TRAUMA-INFORMED CASE MANAGEMENT SERVICES. THIS ENHANCED LEVEL OF SUPPORT ALLOWED STAFF TO PROVIDE INDIVIDUALIZED ASSISTANCE TAILORED TO PARTICIPANT NEEDS, HELPING YOUNG ADULTS ADDRESS BARRIERS RELATED TO HOUSING STABILITY, EDUCATION, EMPLOYMENT, MENTAL HEALTH, AND INDEPENDENT LIVING WHILE INCREASING ENGAGEMENT AND SERVICE ACCESSIBILITY.
- EXPANSION OF COMMUNITY PARTNERSHIPS AND COORDINATED SERVICE DELIVERY - FPC HOPE CENTER SIGNIFICANTLY EXPANDED ITS COLLABORATIVE NETWORK BEYOND ITS INITIAL PILOT PARTNERSHIP MODEL. THE ORGANIZATION ESTABLISHED AND STRENGTHENED RELATIONSHIPS WITH MULTIPLE COMMUNITY PARTNERS, INCLUDING SCHOOL SYSTEMS, EMERGENCY SHELTERS, AND THE KANAWHA VALLEY COLLECTIVE CONTINUUM OF CARE PARTNERS. THESE COLLABORATIONS IMPROVED REFERRAL PATHWAYS, INCREASED ACCESS TO SUPPORTIVE SERVICES, AND ENHANCED COORDINATED CARE FOR PROGRAM PARTICIPANTS THROUGHOUT THE COMMUNITY.
