Program accomplishments

  • The Clinic operates a standalone free clinic with an in-house pharmacy located in Cabarrus County. The Clinic provides health care and pharmacy services to low-income, uninsured, working aged adults who have chronic health conditions (i.e., high blood pressure, diabetes, heart disease) and other primary medical care needs. Clinical and pharmacy services are provided three days and one evening each week. During the year ended June 30, 2025, the Clinics records indicate the Clinic served 682 clinic patients, including 55 new patients, with 3953 appointments. Services included labwork, primary care checkups, sick visits, chronic disease management, specialty appointments, dental health, behavioral health, and eye health. Partnerships with local medical providers, universities, and nonprofit organizations allow us to provide behavioral health and dental care. Patients also received healthcare coaching, care coordination, and referrals for social determinants of health. Our patients share that they experience food and housing insecurity, an interest in finding work, lack of transportation, and domestic violence. Partnerships with local nonprofit organizations allow us to refer patients for this ongoing support, and our Patient Services team provides this coordination. In addition to Clinic and Care Coordination for the Clinic, the Clinic provides patient eligibility for the countys health safety net HealthNet Cabarrus, hospital and partner providers who provide specialty care. During the year ended June 30, 2025, 1,061 individuals were enrolled in HealthNet Cabarrus.
