Program accomplishments

  • THE COMMUNITY SUPPORT PROGRAM PROVIDES FOOD, NUTRITION, RESOURCE NAVIGATION, AND OTHER SUPPORTIVE SERVICES TO INDIVIDUALS AND FAMILIES EXPERIENCING HUNGER, FOOD INSECURITY, AND ECONOMIC HARDSHIP THROUGHOUT MERCER COUNTY. THE GUILD OPERATES ITS OPEN CHOICE FOOD PANTRY IN PARTNERSHIP WITH CATHOLIC CHARITIES, DIOCESE OF TRENTON, AT ITS CAMPUS AT 73 N. CLINTON AVENUE IN TRENTON. DURING FY 2024-2025, THE PANTRY RECORDED 18,117 HOUSEHOLD VISITS, BENEFITING 54,351 INDIVIDUALS. IN ADDITION TO REGULAR PANTRY SERVICES, THE PROGRAM PROVIDES TARGETED NUTRITION ASSISTANCE AND SEASONAL SUPPORT. TWELVE SENIORS PARTICIPATE IN THE USDA COMMODITY SUPPLEMENTAL FOOD PROGRAM AND RECEIVE SUPPLEMENTAL NUTRITIOUS FOOD EACH MONTH. DURING THE ANNUAL THANKSGIVING DISTRIBUTION, THE GUILD PROVIDED 1,490 FAMILIES WITH TURKEYS AND ACCOMPANYING MEAL INGREDIENTS. THROUGH ITS CHRISTMAS TOY AND SENIOR INITIATIVE, THE GUILD SERVED 796 INDIVIDUALS, INCLUDING 562 CHILDREN AND 234 SENIORS, WITH TOYS, GIFTS, AND SEASONAL SUPPORT.

  • THE HOME HEALTH NURSING PROGRAM PROVIDES FREE, PROFESSIONAL IN-HOME NURSING SERVICES TO LOW-INCOME, HOMEBOUND ADULTS AGE 60 AND OLDER THROUGHOUT MERCER COUNTY. THE PROGRAM IS ACCREDITED BY THE NATIONAL INSTITUTE FOR HOME CARE ACCREDITATION (NIHCA). MOUNT CARMEL GUILD DOES NOT BILL MEDICARE OR OTHER INSURANCE FOR THESE SERVICES, ALLOWING VULNERABLE SENIORS WHO MAY OTHERWISE FACE BARRIERS TO CARE TO RECEIVE ESSENTIAL NURSING SUPPORT AT NO COST. DURING FY 2024-2025, REGISTERED NURSES PROVIDED 477 UNITS OF CARE, WITH EACH UNIT REPRESENTING UP TO 45 MINUTES OF DIRECT SERVICE. NURSING SERVICES INCLUDED COMPREHENSIVE ASSESSMENTS, MEDICATION MONITORING AND MANAGEMENT, CHRONIC-DISEASE MANAGEMENT, VITAL-SIGN MONITORING, BLOOD-SUGAR TESTING, INJECTIONS, WOUND CARE, SAFETY ASSESSMENTS, HEALTH EDUCATION, AND COORDINATION WITH PHYSICIANS, PHARMACIES, FAMILY CAREGIVERS, CASEWORKERS, AND OTHER SERVICE PROVIDERS. NEARLY HALF (47.1%) OF PATIENTS HAD BEEN ENROLLED IN THE PROGRAM FOR MORE THAN FOUR YEARS, REFLECTING THE CONTINUITY OF CARE THE PROGRAM PROVIDES TO SENIORS WITH ONGOING HEALTH AND SUPPORT NEEDS. THE PROGRAM'S CENTRAL GOAL IS TO HELP PATIENTS REMAIN MEDICALLY STABLE AND CONTINUE LIVING SAFELY AND INDEPENDENTLY IN THEIR OWN HOMES FOR AS LONG AS POSSIBLE. NURSES MONITOR CHANGES IN HEALTH STATUS, REINFORCE MEDICATION AND TREATMENT PLANS, IDENTIFY SAFETY OR SOCIAL-SERVICE CONCERNS, AND COMMUNICATE WITH MEDICAL PROVIDERS AND FAMILY MEMBERS WHEN INTERVENTION IS NEEDED. PATIENTS REQUIRING ADDITIONAL SOCIAL OR COMMUNITY- BASED SUPPORT ARE LINKED TO APPROPRIATE RESOURCES AND SERVICE PROVIDERS. THROUGH CONSISTENT NURSING OVERSIGHT, EDUCATION, ADVOCACY, AND CARE COORDINATION, THE PROGRAM SUPPORTS AGING IN PLACE WITH DIGNITY WHILE HELPING REDUCE RISKS THAT CAN LEAD TO AVOIDABLE EMERGENCY-ROOM VISITS, HOSPITALIZATION, OR PREMATURE INSTITUTIONAL PLACEMENT.
  • THE GUILD ALSO OPERATES DREAM AGAIN TRENTON, AN INITIATIVE DESIGNED TO CONNECT CARE RECEIVERS WITH OPPORTUNITIES THAT SUPPORT PERSONAL GROWTH, PROFESSIONAL DEVELOPMENT, AND GREATER LONG-TERM STABILITY. THROUGH DREAM AGAIN TRENTON, MOUNT CARMEL GUILD PARTNERS WITH COMMUNITY AGENCIES AND SERVICE PROVIDERS TO MAKE REFERRALS FOR WORKFORCE READINESS, EMPLOYMENT AND CAREER DEVELOPMENT, EDUCATION AND SKILLS-BUILDING, PUBLIC- BENEFITS EDUCATION, DIGITAL ACCESS, HOUSING AND HEALTH-RELATED RESOURCES, FINANCIAL LITERACY, AND OTHER SUPPORTIVE SERVICES. THE INITIATIVE EXPANDS THE GUILD'S ROLE BEYOND MEETING IMMEDIATE NEEDS BY HELPING CARE RECEIVERS CONNECT WITH TRUSTED PARTNERS AND RESOURCES THAT CAN STRENGTHEN SELF- SUFFICIENCY AND IMPROVE QUALITY OF LIFE. HOLIDAY PROJECTS, FOOD COLLECTIONS, AND YEAR-ROUND SERVICES ARE MADE POSSIBLE THROUGH THE GENEROUS SUPPORT OF LOCAL PARISHES, SCHOOLS, CORPORATIONS, FOUNDATIONS, CIVIC ORGANIZATIONS, VOLUNTEERS, AND COMMUNITY PARTNERS. TOGETHER, THESE PARTNERSHIPS ENABLE MOUNT CARMEL GUILD TO RESPOND TO IMMEDIATE NEEDS WHILE PRESERVING DIGNITY AND CREATING PATHWAYS TOWARD GREATER HOUSEHOLD STABILITY.
