- ISPY 2.2 - I-SPY 2.2 USES A 'SEQUENTIAL MULTIPLE ASSIGNMENT RANDOMIZED TRIAL' (SMART) DESIGN - THAT IS BOTH SMART AND ADAPTIVE. THE TRIAL CONSISTS OF UP TO THREE BLOCKS OF TREATMENT. THE FIRST, BLOCK A, INCLUDES NOVEL TARGETED THERAPIES, WITHOUT STANDARD CHEMOTHERAPY. IF A PATIENT ACHIEVES A COMPLETE RESPONSE AFTER BLOCK A, THEY CAN AVOID TOXIC CHEMOTHERAPY AND GO DIRECTLY TO SURGERY. IF THEY HAVE RESIDUAL CANCER, OR THE TREATMENT IS FOUND TO HAVE LITTLE EFFECT HALFWAY THROUGH THE COURSE OF TREATMENT, THEY ARE RECOMMENDED TO PROCEED TO TREATMENT IN BLOCK B. IN BLOCK B, PATIENTS ARE ASSIGNED A TREATMENT REGIMEN MOST LIKELY TO WORK FOR THEIR SUBTYPE, OFTEN COMBINED WITH CHEMOTHERAPY SINCE THE RISK OF TOXICITY IS MORE ACCEPTABLE IN PATIENTS WITH MORE RESISTANT CANCER. (CONTINUED ON SCHEDULE O)IF BLOCK B TREATMENT IS EFFECTIVE, THE PATIENT CAN SKIP ADDITIONAL CHEMOTHERAPY AND GO TO SURGERY. ONLY IF THEY STILL HAVE TUMOR LEFT AFTER BLOCK B, WILL THE PATIENTT GET RESCUE THERAPY WITH DOXORUBICIN AND CYCLOPHOSPHAMIDE A CHEMOTHERAPY REGIMEN THAT HAS BEEN AROUND FOR DECADES. (CONTINUED ON SCHEDULE O)A KEY ELEMENT IN REDUCING OVER TREATMENT OF DCIS IS IDENTIFYING BIOMARKERS THAT REFLECT THE RISK OF PROGRESSION SO THAT THOSE WITH LOW-RISK LESIONS ARE SPARED SURGERY. RECAST DCIS FEATURES THE ASSESSMENT OF MRI IMAGING AND MOLECULAR-BASED BIOMARKER ANALYSES IN ADDITION TO EVALUATING NEW INVESTIGATIONAL AGENTS IN THIS SETTING.
