Program accomplishments

  • NORTHERN DUTCHESS HOSPITAL (NDH) IS AN 84-BED NONPROFIT ACUTE CARE HOSPITAL LOCATED IN RHINEBECK, NEW YORK. THE HOSPITAL IS COMMITTED TO DELIVERING HIGH-QUALITY, PATIENT-CENTERED, AND MISSION-DRIVEN CARE TO THE COMMUNITIES IT SERVES. PATIENTS HAVE ACCESS TO COMPREHENSIVE, CLOSE-TO-HOME SERVICES RANGING FROM PREVENTIVE CARE FOR CANCER AND HEART DISEASE TO BARIATRIC SURGERY, ORTHOPEDIC SERVICES, AND HEALTHY AGING PROGRAMS. NDH HAS ESTABLISHED A STRONG REPUTATION IN THE HUDSON VALLEY, PARTICULARLY FOR EXCELLENCE IN MATERNITY AND ORTHOPEDIC CARE. NORTHERN DUTCHESS HOSPITAL IS PART OF NUVANCE HEALTH, WHICH IN MAY 2025 JOINED NORTHWELL HEALTH, CREATING AN EXPANDED INTEGRATED HEALTH SYSTEM THAT ENHANCES ACCESS TO COORDINATED, HIGH-QUALITY CARE ACROSS NEW YORK'S HUDSON VALLEY AND WESTERN CONNECTICUT. AS PART OF NORTHWELL HEALTH, NEW YORK'S LARGEST HEALTH SYSTEM, NDH PATIENTS BENEFIT FROM EXPANDED CLINICAL COORDINATION, ENHANCED SPECIALTY EXPERTISE, AND ACCESS TO SYSTEM-WIDE BEST PRACTICES, WHILE MAINTAINING A STRONG FOCUS ON COMMUNITY-BASED CARE. THROUGH THIS INTEGRATED SYSTEM, NDH OFFERS A BROAD RANGE OF SERVICES, INCLUDING BARIATRIC SURGERY AND MEDICAL WEIGHT LOSS, CRITICAL CARE, DIABETES EDUCATION, EMERGENCY SERVICES, HEART AND VASCULAR CARE, IMAGING AND RADIOLOGY, INFUSION THERAPY, NEUROLOGY AND NEUROSURGERY, ORTHOPEDIC CARE, PHYSICAL REHABILITATION, PRIMARY CARE, RHEUMATOLOGY, SLEEP MEDICINE, URGENT CARE, WOMEN'S HEALTH, BEHAVIORAL HEALTH, CANCER CARE, DENTISTRY, DIGESTIVE HEALTH, ENDOCRINOLOGY, GENETIC COUNSELING, HOME HEALTH SERVICES, INFECTIOUS DISEASE CARE, NEPHROLOGY, OCCUPATIONAL MEDICINE, PATIENT BLOOD MANAGEMENT, PEDIATRICS, PODIATRY, PULMONARY CARE, GERIATRIC CARE, SURGICAL SERVICES, UROLOGY, AND WOUND CARE AND HYPERBARIC MEDICINE. NDH MAINTAINS A STRONG COMMITMENT TO CLINICAL QUALITY AND PATIENT SAFETY AND HAS BEEN RECOGNIZED BY SEVERAL NATIONAL ORGANIZATIONS. THE HOSPITAL WAS NAMED A TOP TEACHING HOSPITAL BY THE LEAPFROG GROUP FOR OUTSTANDING QUALITY AND SAFETY AND RECEIVED A FIVE-STAR QUALITY RATING FROM THE CENTERS FOR MEDICARE & MEDICAID SERVICES (CMS), PLACING IT AMONG A SELECT GROUP OF HOSPITALS NATIONWIDE. IT HAS ALSO BEEN DESIGNATED AS A ROBOTIC CENTER OF EXCELLENCE BY THE SURGICAL REVIEW CORPORATION. IN 2025, U.S. NEWS & WORLD REPORT RECOGNIZED NDH AS "HIGH PERFORMING" IN HEART FAILURE, AND THE HOSPITAL RECEIVED THE AMERICAN HEART ASSOCIATION'S GET WITH THE GUIDELINES STROKE GOLD PLUS AWARD. ADDITIONAL RECOGNITION INCLUDES MULTIPLE HEALTHGRADES FIVE-STAR RATINGS FOR CLINICAL OUTCOMES IN AREAS SUCH AS HIP FRACTURE TREATMENT, HEART FAILURE, SEPSIS, GASTROINTESTINAL BLEEDING, AND DIABETIC EMERGENCIES. THESE RECOGNITIONS REFLECT THE HOSPITAL'S COMMITMENT TO QUALITY IMPROVEMENT, PATIENT SAFETY, AND THE DELIVERY OF EFFECTIVE, EVIDENCE-BASED CARE. NORTHERN DUTCHESS HOSPITAL CONDUCTS A COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA) EVERY THREE YEARS TO IDENTIFY THE HEALTH NEEDS, BARRIERS, AND PRIORITIES OF THE COMMUNITIES IT SERVES. THE FINDINGS INFORM THE DEVELOPMENT OF THE COMMUNITY HEALTH IMPROVEMENT PLAN (CHIP), WHICH OUTLINES TARGETED STRATEGIES AND MEASURABLE ACTIONS TO ADDRESS IDENTIFIED NEEDS. BASED ON THE MOST RECENT CHNA, NDH FOCUSED ITS COMMUNITY BENEFIT ACTIVITIES ON TWO PRIORITY AREAS: PREVENTING CHRONIC DISEASE AND PROMOTING WELL-BEING WHILE ADDRESSING MENTAL HEALTH AND SUBSTANCE USE. IN SUPPORT OF CHRONIC DISEASE PREVENTION, NDH OFFERS THE CENTERS FOR DISEASE CONTROL AND PREVENTION (CDC) NATIONAL DIABETES PREVENTION PROGRAM (DPP), AN EVIDENCE-BASED LIFESTYLE INTERVENTION DESIGNED TO REDUCE THE RISK OF TYPE 2 DIABETES. THE PROGRAM HAS ACHIEVED "FULL PLUS RECOGNITION" FROM THE CDC, DEMONSTRATING STRONG PARTICIPANT OUTCOMES AND PROGRAM EFFECTIVENESS. TO SUPPORT BEHAVIORAL HEALTH AND SUBSTANCE USE PREVENTION, NDH PROVIDES ACCESS TO MENTAL HEALTH FIRST AID, A NATIONALLY RECOGNIZED, EVIDENCE-BASED TRAINING PROGRAM THAT EQUIPS INDIVIDUALS WITH THE SKILLS TO RECOGNIZE AND RESPOND TO SIGNS OF MENTAL ILLNESS AND SUBSTANCE USE DISORDERS. THIS PROGRAM HAS BEEN EXPANDED ACROSS HOSPITAL COMMUNITIES IN BOTH NEW YORK AND CONNECTICUT TO ENSURE BROADER ACCESS TO THIS CRITICAL RESOURCE. ADDITIONAL COMMUNITY BENEFIT INITIATIVES INCLUDE TRANSPORTATION ASSISTANCE, FITNESS AND WELLNESS PROGRAMMING, HEALTH-RELATED SOCIAL NEEDS SCREENINGS, AND DISTRIBUTION OF NALOXONE (NARCAN) AND FENTANYL TEST STRIPS. NDH ALSO PROVIDES COMMUNITY EDUCATION ON TOPICS SUCH AS HEALTHY LIFESTYLE BEHAVIORS, STROKE PREVENTION, CANCER SCREENING, SMOKING CESSATION, CHRONIC DISEASE SELF-MANAGEMENT, AND STRESS MANAGEMENT, OFTEN IN PARTNERSHIP WITH COMMUNITY-BASED ORGANIZATIONS, SCHOOLS, AND SENIOR CENTERS. PARTICIPATION IN NATIONAL DRUG TAKE-BACK PROGRAMS HAS RESULTED IN THE SAFE DISPOSAL OF SIGNIFICANT AMOUNTS OF MEDICATIONS AND SHARPS ACROSS THE SERVICE AREA. OVER THE PAST SEVERAL YEARS, STAFF HAVE ALSO CONTRIBUTED TIME AND EXPERTISE TO NUMEROUS COMMUNITY ORGANIZATIONS AND TASK FORCES, FURTHER SUPPORTING COMMUNITY HEALTH AND WELL-BEING. THESE INITIATIVES ARE DESIGNED TO ADDRESS IDENTIFIED COMMUNITY HEALTH NEEDS, REDUCE BARRIERS TO CARE, AND IMPROVE HEALTH OUTCOMES, PARTICULARLY AMONG VULNERABLE AND UNDERSERVED POPULATIONS. IN FY25, NORTHERN DUTCHESS HOSPITAL HAD 25,243 PATIENT DAYS AND 6,213 DISCHARGES IN ACUTE CARE. THERE WERE 24,265 IN EMERGENCY ROOM VISITS AND 79,496 IN OTHER OUPATIENT VISITS/PROCEDURES.
