THE ORGANIZATION PROVIDES MOTOR VEHICLE TRANSPORTATION TO PERSONS WITH DISABILITIES SERVED BY UNITED CEREBRAL PALSY ASSOCIATION OF NEW YORK STATE, INC (DBA CONSTRUCTIVE PARTNERSHIPS UNLIMITED, INC), THE SOLE CORPORATE MEMBER OF CEREBRAL PALSY TRANSPORT, INC.
