<Desc>Deaconess Illinois Crossroads, Inc. (Hospital), is a 47-bed acute care hospital located in Mount Vernon, Illinois, formerly known as Crossroads Community Hospital. The Hospital provides inpatient, outpatient, and emergency care services for residents of southern Illinois. Crossroads Community Hospital was acquired by Deaconess Regional Healthcare Services Illinois, Inc. (Deaconess) which is organized as a nonprofit corporation to facilitate diversification of services across Southern Illinois and Indiana.(Statement continued on Schedule O)The Hospital provides care to patients who meet certain criteria under its charity care policy without charge or at amounts less than established rates. Since the Hospital does not pursue collections of these amounts, they are not reported as patient service revenue. The estimated cost of providing these services was $237,000 for the year ended September 30, 2025, calculated by multiplying the ratio of cost to gross charges for the Hospital by the gross uncompensated charges associated with providing charity care to its patients.</Desc>

<ExplanationTxt>Part V, Section B, Line 11: Deaconess Illinois Crossroads addresses the health needs identified in its 2024 Community Health Needs Assessment (CHNA), the hospital's first CHNA following its transition to not-for-profit status as part of Deaconess Health System. The three priority needs identified through that process are: Access to Care; Community Collaboration; and Health Education. Implementation activities during FY25 (October 1, 2024 through September 30, 2025) are described below by priority area.Priority 1: Access to CareThe 2024 CHNA called for improved access to care through the recruitment and retention of providers for primary care, specialty care, mental health and substance use treatment, along with evaluation of internal processes to ensure the best possible patient experience. Crossroads expanded diagnostic and specialty service capacity during FY25. The hospital launched PET services to complement its oncology program, installed a new CT unit with low-dose lung cancer screening capability, and continued to offer a full range of breast health diagnostics including screening mammography, diagnostic mammography, stereotactic breast biopsy and ultrasound-guided breast biopsy. Prostate MRI and prostate ultrasound are also available locally. The hospital hired an imaging navigator during FY25 to manage and coordinate screening exams, reducing barriers for patients moving between screening, diagnostic follow-up and treatment. To improve the customer experience, Crossroads implemented a formal Leader Rounding program to strengthen patient engagement, identify concerns in real time and improve service recovery. Patient experience was monitored through Press Ganey surveys, leader rounding feedback, online reviews and social media engagement to identify trends and improvement opportunities. The hospital reestablished its Patient Experience Advisory Council, which includes physicians, staff, auxiliary members, advisory board representatives and community members, to provide feedback and guide patient-centered improvements. Bedside shift report and high reliability rounding practices were reinforced to improve communication, patient involvement in care, safety and overall experience.Evaluation of mental health and substance use service gaps and provider retention efforts remain ongoing components of the adopted implementation strategy, coordinated with Deaconess Health System and community partners. Priority 2: Community CollaborationThe 2024 CHNA identified community resource coordination as a top opportunity and called for participation in or development of an inter-agency group, investigation of a shared resource guide, and support for local agencies addressing food insecurity. Supporting data showed 47% of the service area population living in a USDA-recognized food desert across 13 census tracts, with 22.63% of low-income residents also experiencing low food access.Rather than create a duplicative structure, Crossroads determined that an active coalition already served this function and participated in the Mt. Vernon area MACC group, a community collaboration and networking coalition that convenes organizations regularly to share resources, programs and community services. A Crossroads case management representative attends these meetings.Crossroads likewise determined that a new resource guide was unnecessary. The hospital instead directs agencies and residents to the Caregiver Support Guide published by the Midland Area Agency on Aging for Jefferson and Clinton counties, with companion guides available in the same format from Area Agencies on Aging serving surrounding counties, and to www.findhelpsi.org, a comprehensive resource directory launched during the year. The hospital provided ongoing meal support to two local shelters throughout FY25. Approximately 25 meals were delivered monthly to the Lifeboat Alliance Family Shelter on the fourth Friday of each month, and 20 to 30 meals were delivered to the Family Life Shelter on the first Tuesday of each month, at an estimated value of $175 per month per shelter. All meals were prepared by hospital Dietary staff and coordinated by Marketing.Employees supported additional food security efforts through a November canned food drive benefiting Angels on Assignment, promoted to all Crossroads staff. Hospital staff volunteered at the Angels on Assignment food pantry on the first Friday of the month during October, November and December, and volunteered one day per month from June through August with the District 80 Sun Meals 2 Go program, distributing groceries and meals to Jefferson County families.Priority 3: Health EducationFollowing the September 2024 transition from MedHost to Epic, nursing staff received education on using Epic patient education resources, including materials available in multiple languages to better serve diverse patient populations. Patient education workflows were standardized within Epic to support consistent delivery of evidence-based education across the care continuum, with continued emphasis on teach-back methods and patient-specific education to support health literacy.Nursing leaders engaged with community partners serving behavioral health and homeless populations to better understand community health needs and barriers to care, informing patient education strategies and resource connections.Crossroads hosted a community health fair on April 22, 2025, at the Rolland Lewis Building in Mt. Vernon from 10:00 a.m. to 2:00 p.m. The event featured 13 hospital service lines, five screenings offered to the public including free PSA testing for men, and participation by external community vendors. The hospital increased access to free and low-cost cancer screenings. Breast Check Fridays offered walk-in mammograms at all three Deaconess Illinois hospital sites during October 2024, supported by a Facebook campaign and themed activities in Imaging, with MD Save pricing options promoted. Cancer Screening by Age fliers were posted at all primary care clinics including the rural health clinic. Marketing promoted preventive services and early detection through educational campaigns covering primary care, mammography, mobile mammography, colon cancer screening, lung cancer screening, prostate cancer detection, kidney health, cardiac rehabilitation and pulmonary rehabilitation.Crossroads pursued school and college partnerships supporting health education and career exposure. The hospital participated in a Rend Lake College partnership shared across the Deaconess Illinois hospitals, with an athletic trainer provided through Deaconess Illinois Occupational Health. In connection with the Great American Smokeout, tobacco cessation education developed by Case Management was presented to more than 300 HOSA students at Mt. Vernon Township High School, supported by 1,000 silicone awareness bracelets.Stroke awareness education was delivered through three channels during FY25: a BE FAST social media campaign during Stroke Awareness Month in May 2025, BE FAST education presented by the Neurology Center of Excellence at the April health fair, and BE FAST education delivered at a May 2025 Lunch and Learn at the Rolland Lewis Building.</ExplanationTxt>
