PathForward supports approximately 200 scattered-site housing placements across Arlington County through PSH, Rapid Re-Housing, and Transitional Housing programs. Clients receive trauma-informed case management, healthcare coordination, benefits navigation, income support, and assistance with activities of daily living. The Mobile Medical Program (MMP) delivers low-barrier, trauma-informed medical and behavioral health care directly to individuals living unsheltered, in emergency shelter, and in Permanent Supportive Housing. The Mobile Medical "Dream Team" model is interdisciplinary and includes a nurse practitioner or registered nurse, a licensed clinical social worker, and a case manager. The team provides wound care, chronic disease monitoring (including blood pressure and glucose checks), medication management and prescription coordination, behavioral health assessment and therapy, crisis intervention, overdose prevention education (including NARCAN administration and training), and field-based clinical interventions. Services are delivered through street outreach, shelter-based clinics, and in-home visits for medically fragile residents.
