Form 990, Part VI, Section B, Line 11b Form 990 is prepared by an independent accountant. The organization provides a full draft of Form 990 to all board members prior to filing. Board members have an opportunity to provide input and ask questions about the 990 prior to filing. Form 990, Part VI, Section B, Line 12c Annually, all board members are required to review the Conflict of Interest Policy, and complete and sign a Conflict of Interest Questionnaire, in which they identify potential conflicts.
