Services for adults help decrease overall family homelessness, while giving the children the skills needed to break the future cycle. IHS is committed to promoting national and county-wide objectives by providing low-income homeless families with stable/safe housing, as well as supporting expanded economic and employment opportunities. Goals are clearly defined and include: increased income (employment), stable permanent housing, long-term personal growth, and eventual self-sufficiency. 0 0 8570786 2549055 1589909 In 2025, Inland Housing Solutions prioritized strategic growth, program innovation, and the continued delivery of high-quality services. As a recipient of Homeless Housing, Assistance and Prevention (HHAP) Round 4 funds, IHS sought to replicate the success of its legacy Rapid Re-housing program by launching a permanent housing program targeted to seniors and adults with disabilities. Permanent housing services include housing navigation, move-in and rental assistance, and intensive case management focused on housing stability, social drivers of health, and improved clinical outcomes. Since the program's launch in November 2025, IHS has housed two senior households through the HHAP-4 program. The continued expansion of CalAIM initiatives, particularly Enhanced Care Management (ECM) and Community Supports (CS), enabled IHS to broaden its direct service offerings, including care coordination, behavioral health services, and individualized housing navigation. The Agency continued serving an expanded target population across Riverside and San Bernardino Counties, including individual households who are homeless or at risk of homelessness, as well as medically fragile, elderly, and disabled individuals. Across all programs, IHS served more than 2,000 households, providing housing services, care coordination, case management, and behavioral health support.Program highlights include:Rapid Re-housing: Housing Navigation, permanent housing, move-in and rental assistance, and intensive case management for 36 families with children. Permanent Supportive Housing: In partnership with Inland Empire Health Plan (IEHP), rental subsidy, tenancy support and supportive services for 144 formerly chronically homeless households.Community Supports: In partnership with IEHP, provided non-medical, medically appropriate services to 1,306 households who were homeless or at risk of homelessness and living with chronic health conditions. Services included housing navigation, tenancy support, home modifications, and housing deposits.Enhanced Care Management: In partnership with IEHP, delivered comprehensive health assessments, care coordination, and care coordination healthcare services to 502 Medi-Cal members.Housing Outcomes: Across all programs, IHS successfully housed 157 households in permanent housing.In 2025, IHS continued to coordinate the multi-agency Countywide Housing Search effort. Through partnerships with private property owners, the Agency identified suitable housing opportunities for individuals and families connected to rental subsidies and community-based supportive services. Property owners remain essential partners in expanding access to rental housing for vulnerable households and advancing regional efforts to reduce homelessness.IHS collaborates closely with multiple service providers through San Bernardino County's 211 Coordinated Entry System (CES) to facilitate housing identification and placement while maintaining strong relationships with property owners and operators.
