<MissionDesc>Georgia Transplant Foundation (GTF) exists to bridge the financial, educational, and emotional gaps faced by organ transplant candidates, recipients, living donors, and their families throughout Georgia. In 2025, GTF continued to provide a comprehensive portfolio of programs and services designed to improve access to transplantation, support long-term recovery, and promote financial stability for those impacted by organ failure and transplantation.Throughout the year, GTF provided direct financial assistance, educational programming, mentoring and peer support services, and return-to-work readiness support through the JumpStart Program. The organization also expanded its outreach and educational footprint across Georgia through multiple Trends in Transplant (TNT) Conferences and community-based engagement initiatives designed to connect patients, caregivers, social workers, and medical professionals with critical transplant-related resources.</MissionDesc>

<Desc>Financial Assistance Programs:Georgia Transplant Foundation's Financial Assistance Programs provide critical short-term support to transplant candidates, recipients, and living donors across Georgia by addressing financial burdens that often jeopardize access to care, treatment adherence, or post-transplant recovery. Through its Direct Financial Assistance Programs and Transplant Fundraising Program (TFP), GTF helped patients and families offset transplant-related expenses not covered by insurance or other sources.Assistance provided through these programs included support for immunosuppressant medications, insurance premiums, transportation, temporary lodging, dental assistance, transplant evaluations, and emergency financial needs directly impacting transplant stability and recovery. GTF also continued to provide guidance and matching support opportunities through the Transplant Fundraising Program to help eligible patients prepare for and sustain post-transplant care. Each request for assistance remained needs-based and required financial hardship verification, transplant center or social worker validation, and proof of Georgia residency. These programs served as a vital safety net for transplant patients and families experiencing medical and financial crises, helping reduce barriers to care while allowing patients to focus on recovery, stability, and long-term health outcomes.</Desc>

<Desc>Program Development & Strategic Growth:In 2025, Georgia Transplant Foundation continued strengthening and refining its programs and operational infrastructure to better meet the evolving needs of Georgia's transplant community. The Organization streamlined several internal processes to improve efficiency, patient accessibility, and long-term sustainability while maintaining its commitment to compassionate, individualized support.GTF continued expanding awareness of its programs and services beyond metro Atlanta through targeted outreach to transplant centers, dialysis clinics, social workers, and healthcare providers across the state. Educational initiatives and outreach efforts also focused on increasing engagement within African American communities and underserved populations disproportionately impacted by chronic illness and organ failure. The Organization further strengthened its focus on financial self-sufficiency and long-term patient stability through continued development of the JumpStart Program, which assists transplant patients with return-to-work readiness, career support, and pathways toward greater financial independence following transplantation. In addition, GTF advanced several strategic initiatives during 2025, including technology modernization projects, enhanced data collection and reporting capabilities, and planning efforts designed to support future statewide growth and program expansion. These efforts aligned with GTF's mission of bridging the gap between hope and opportunity for Georgia's transplant community.</Desc>

<Desc>Educational & Community Engagement Initiatives:During 2025, GTF continued hosting Trends in Transplant (TNT) Conferences and other educational initiatives designed to provide patients, caregivers, living donors, and healthcare professionals with access to transplant-related education, peer connection, and supportive resources. These conferences addressed a wide range of topics, including financial preparedness, post-transplant wellness, mental health, medication adherence, insurance navigation, and long-term quality of life. GTF also continued building strategic relationships with healthcare providers, community organizations, corporate partners, and transplant advocates throughout Georgia to increase awareness of available resources and strengthen support systems for transplant patients and families statewide.</Desc>

<FormAndLineReferenceDesc>Part II, Section C, Line 17A Facts and Circumstances Test:</FormAndLineReferenceDesc>
<ExplanationTxt>Explanation: While the Foundation does not qualify as publicly supported under the mechanical 33 1/3 percent-of-support test set forth in the IRS Regulations, the Foundation should be considered a publicly supported charity under the facts and circumstances as set forth in the IRS Regulations. Under the facts and circumstances test, an organization will be treated as publicly supported if it establishes, under all the facts and circumstances, that (i) it normally receives a substantial part of its support from government units or direct or indirect contributions from the general public, and (ii) it is of the nature of a "publicly supported organization, taking into account the factors described in the Regulations. As described below, the Foundation submits that it satisfies the facts and circumstances test for recognition as a publicly supported organization. (1) The Foundation has ongoing and continuous programs to solicit support from the general public, transplant patients and other not-for-profit organizations. These programs include a direct mail fundraising campaign, solicitation on its website and other various social media outlets. The scope of the Foundation's solicitation program is reasonable in light of its focus on providing assistance to organ transplant patients. (2) The IRS Regulations provide that the fact that an organization meets the requirements through support from governmental units or directly or indirectly from a representative number of persons, rather than receiving almost all of its support from the members of a single family, will be taken into consideration in determining whether an organization is "publicly supported." In determining what is a "representative number of persons," consideration will be given to the type of organization involved, the length of time it has been in existence, and whether it limits its activities to a particular community or region or to a special field which can be expected to appeal to a limited number of persons. The Foundation's support is derived from a variety of sources, including investment income and contributed income. (3) The IRS Regulations provide that the fact that an organization has a governing body which represents the broad interests of the public, other than the personal or private interests of a limited number of donors (or persons standing in a relationship to such donors) will be taken into account in determining whether an organization is "publicly supported." An organization will be treated as meeting this requirement if it has a governing body which is composed of public officials acting in their capacities as such, of individuals selected by public officials acting in their capacities as such, of persons having special knowledge or expertise in the particular field or discipline in which the organization is operating, of community leaders, such as elected or appointed officials, clergymen, educators, civic leaders or other such persons representing a broad cross-section of the views and interests of the community, or, in the case of a membership organization, of individuals elected pursuant to the organization's governing instrument or by-laws by a broadly-based membership. The Foundation's Board of Directors is separate and diverse. The Board members come from a geographically dispersed area around the State of Georgia and, thus, represent a broad cross-section of the views and interests of those served by the Foundation. (4) The IRS Regulations contain a series of provisions not applicable to the Foundation because the Foundation is not an organization of a type that generally provides facilities or services directly for the benefit of the general public on a continuing basis, the Foundation is not an educational or research institution, and the nature of the Foundation's activities and support are different from the nature of the organizations described in these sections. However, as described in the Regulations, the Foundation conducts a definitive program to accomplish its charitable work in the communities of those it serves. The Foundation's programs provide financial and medical assistance to organ transplant patients throughout the state of Georgia. In summary, the Foundation should be and requests that it be classified as a publicly supported charity.</ExplanationTxt>
