The grant is part of the broader Carolyn Adams Ticket for the Cure initiative, which funds breast cancer awareness, education, and support services in Illinois. Community health workers work closely with our local hospitals, clinics and cancer resource centers and our team will work with coordinating communications and building rapport amongst clients and healthcare systems. We utilize our partnerships with healthcare providers, clinics, hospitals, and community organizations to facilitate access to healthcare services, coordinate care, and address social determinants of health. This maternity program is a part of the Blue Cross Blue Shields Health initiaves program and assists clients in managing their pregnancy and preparing for childbirth.
