Additional Information for Schedule I The preceptor development grant program provides $1,000 grants to osteophatic physciians who volunteer as preceptors for medical students and residents. Applicants may submit only one applicaiton per year and fill out an online application form. The form requires proof of medical license and preceptor activity for the fiscal year. Members or stockholder classes and rights Part VI line 6 Any person duly licensed to practice medicine and surgery as an Osteopathic physician and surgeon in the state of Arizona may be admitted to membership upon application and payment of annual dues. Member election for additional members Part VI line 7a Members elect officers and trusteess at the annual meeting. Governing body decisions Part VI line 7b The House of Delegates shall be the legislative and governing body of the association. Any amendments to policies and procedures, including the by laws, are passed by a majority of board member votes before the house of delegates consider any amendment requests. Form 990 governing body review Part VI line 11 The 990 is prepared by an outside accounting firm. It is then discussed and reviewed by the executive director independently, as well as with the finance and executive committees, to determine if any adjustments are needed. The full board of directors is provided a finalized version prior to filing. Conflict of interest policy compliance Part VI line 12c At each board and committee meeting, if there is a discussion of selecting or engaging a vendor or service provider, all in attendance are asked to recuse themselves from the discussion if there could be a perceived conflict. Annually, the organization reviews and discusses the conflict of interest policy and requests that each bard membr list and acknowledge any known conflicts. Other officer or key employee compensation Part VI line 15b The board may hire and compensate individuals for necessary services rendered to the organization so long as such compensation is reasonable. The board shall determine reasonable compensation amounts based upon compensation paid by similarly situated nonprofits for like services. An individual who is a member of the board who received compensation, directly or indirectly from the organization for services, is precluded from participating in discussions or votes pertaining to their own compensation, currently, all directors are volunteers. Governing documents etc available to public Part VI line 19 The organization will provide, in a timely manner, financial statements and governing documents including its conflict of interest policy when requested in writing or in person.
