SPECIFICALLY, THE MACY'S NORTH STAR RELIEF FUND HAS STRINGENT DOCUMENTATION REQUIREMENTS FOR EACH TYPE OF GRANT REQUEST OTHER THAN EMERGENCY GRANTS THAT ARE DESIGNED TO ENSURE THAT THE GRANT REQUEST IS LEGITIMATE AND THE REQUESTED GRANT IS REIMBURSING THE GRANT RECIPIENT FOR AN EXPENSE OR LOSS THAT WAS ACTUALLY INCURRRED. IN THE CASE OF EMERGENCY GRANTS, THE MACY'S NORTH STAR RELIEF FUND DOESN'T REQUIRE THE GRANT RECIPIENT TO PROVIDE THE NORMAL REQUESTED DOCUMENATION THAT APPLIES TO OTHER GRANT TYPES. EMERGENCY GRANTS ARE ONLY ISSUED AS A RESULT OF NATURAL DISASTERS DECLARED BY FEMA. THE AMOUNT OF EMERGENCY GRANTS ARE LIMITED TO $200 PER RECIPIENT. THE RECIPIENT OF AN EMERGENCY GRANT IS REQUIRED TO DESCRIBE THE NEED FOR ASSISTANCE IN DETAIL AS PART OF THE GRANT APPLICATION PROCESS AND LIVE IN THE IMPACTED AREA. A COMPLETE COPY OF THE PROCEDURES, INCLUDING DOCUMENATION REQUIREMENTS, ARE AVAILABLE UPON REQUEST. Part VI, Section B - Line 12c The Macy's North Star Relief Fund, has a written conflict of interest policy. The Policy was approved by the Board of Directors of Macy's North Star Relief Fund and was made part of the corporate records of the Fund. Annually, each director, principal officer and committee member is required to sign a statement which affirms such person has received a copy of the conflicts of interest policy, has read and understands the policy, has agreed to comply with the policy and understands the Fund is charitable and in order to maintain its federal tax exemption it must engage primarily in activities which accomplish one or more of its tax-exempt purposes. Directors, principal officers and members of a committee with governing board-delegated powers are required to disclose any conflicts of interest. Were there to be any such conflicts, the Director/Officer involved is expected to recuse him or herself from matters relating to the conflict of interest. Part VI, Section B - Line 13 Macy's, Inc., the sole member of Macy's North Star Relief Fund, has effectively established a whistleblower policy through the written guidance made available to employees in its colleague guide, code of good business conduct, and business ethics statements. These documents indicate that employees are encouraged to come forward with credible information on illegal practices or violations of adopted policies of the organization, specify that the organization will protect the individual from retaliation, and identifies the parties to whom such information can be reported. The company has established an office of compliance and has made available a confidential employee hotline for the use of any employee who has a question or information related to a legal or policy issue, including knowledge of policy infractions that should be reported. This policy is also followed by the fund. Part VI, Section B - Line 14 14 Macy's, Inc, the sole member of Macy's North Star Relief Fund, has a written document retention and destruction policy. This policy is also followed by the fund. Part VI, Section C - Line 19 The fund makes its governing documents, conflict of interest policy and financial statements available to the public upon request. Part IV, Section B, line 11b The Form 990 is prepared and then separately reviewed by employees of the corporate tax department of Macy's, Inc. it is then circulated to the officers of the organization for review. The Form 990 is then signed by an officer of the organization.
