We had engaged an external accountant to manage our annual filings and were under the good-faith belief that they had submitted the automatic extension request on our behalf prior to the original deadline. Unfortunately, we later discovered that the extension had not been filed, and we took corrective action to prepare and submit the Form 990. This delay was not due to willful neglect, but rather a miscommunication during a change in accounting personnel. We have since implemented stronger internal controls and documentation procedures to ensure all future filings and extensions are confirmed and tracked internally, regardless of external support.
