Form 990, Part VI, Section A, line 6 The members of Convergence Integrated Care (CIC) are Minnesota-based nonprofit corporations meeting all of the following criteria: A) Affirmatively elect to join CIC by executing the IPA Participation Agreement and make the corresponding capital contribution as determined by the Board of Directors; and B) Satisfy all other membership requirements as laid out in the CIC Articles of Incorporation, Bylaws, IPA Participation Agreement, or as established by the Board.
