Promotes physical health, nutrition counseling and education, and additional health support for aging. In 2024, 41,361 meals were served to our Congregate and Home Delivered clients. 183337 184539 TRANSPORTATION PROGRAM: Provides transportation via bus, van, and private auto for medical, social, nutritional, employment, and personal business. Many elderly clients are frail and require private auto escort and/or wheelchair-lift assistance to access Agency vans and buses. In 2024, the Agency provided 4,511 passenger trips of which 1,613 were for medical appointments (740 for frail elderly requiring private auto transport). The remaining rides were for nutrition, employment, education, and social/recreation. 120609 53147 120957 LEGAL AND BENEFIT COUNSELING: Provides support to individuals and families affected by Alzheimer's Disease and related dementia, including grants to affected individuals and families. Programs include Alzheimer's Family Caregiver Support, Outreach, Information and Assistance, Respite Care, and necessary supplies and services. In 2024, 27 clients received full or partial grants totaling $53,147 via Alzheimer's/Family Caregiver Support Programs. About 315 individuals were assisted with Homestead Tax, Medicare, Medicaid, SSI, SeniorCare, prescription medication, and consumer issues. 17087 17191 Health Promotion and Disease Prevention seminars for seniors.
