The Navigator program provides awareness, education and support for patients from the time of cancer diagnosis through survivorship, with the navigator team, which consists of a nurse navigator and cancer survivors who mentor patients during their cancer journey. During 2024 Project H.E.R. provided free resources and guidance to 200 newly diagnosed patients, hosted 3 monthly cancer support groups (for breast cancer, blood cancer and gynecological cancer patients), provided house cleaning services for 16 patients (totaling $3,240) and emergency financial assistance grants totaling $10,797 (ranging from $105 to $1,000 each) paid directly to vendors for 22 patients to provide assistance with payments for rent, mortgage, car loans and repairs, medical expenses, insurance, and utilities. In addition, $10,000 of gas and grocery cards were purchased and distributed to 120 patients. The Cook for Cancer program paid $20,285 for 864 meals (72 meals per month for about 20 families) for families of local cancer patients who were undergoing chemotherapy treatments of all types of cancer diagnoses, oncology and hematology. These grants, ranging from $300 to $1,000 each and paid directly to vendors such as landlords and phone and utility companies, supported the individuals' urgent financial needs, such as rent and mortgage payments, utility and phone expenses, and child care expenses, that the individuals were unable to pay on account of their medical illnesses, work layoffs, deaths in the family, or other unforeseen family circumstances. 2666 2408 0 Youth Support Programs: The Youth Mental Wellness Program spent $2,666 in 2024 to provide mental health resources and support to children and their families. In the community there are too few mental health practitioners and many young people in need, meaning many families who need quick access to mental health care cannot access it. The Foundation is working to fill some of the gaps through this program. The program continued to support an online Youth Mental Resource Guide that offers current information on mental health providers and resources available to residents of Benton, Linn and surrounding rural counties. The guide serves as a valuable tool for those seeking guidance, support, and mental health support phone lines in various areas such as child abuse/neglect, suicide prevention, LBGTQ support, eating disorders, bullying and cyberbullying, and schizophrenia and psychosis. The guide also lists counseling services that are available for those individuals and families who are uninsured and where to find emergency housing for youths aged 10 to 17. Emergency assistance grants are paid directly to landlords, banks, utility companies and other vendors after recipients provide proof of financial hardship and documentation of their pending bills. Project H.E.R. grants are awarded for patients who are receiving treatment for cancer and need financial assistance with rent, mortgage payments, car loan payments and repairs, insurance, medical bills, phone and utilities bills, house cleaning, groceries, and gasoline. Form 990, Part VI, Section A, Line 2 Two of the board members (Kalah McVay and Cara Wheeler) have a business relationship since they are employed by the same employer. Form 990, Part VI, Section B, Line 11b The finance committee of the Board of Directors reviews Form 990, makes suggested changes that are needed to make the return more accurate, and then makes a recommendation to the full Board prior to filing with the Internal Revenue Service about approving the return as prepared or with recommended revisions. The Board of Directors then reviews and approves the Form 990 for filing. Form 990, Part VI, Section B, Line 12c The Board of Directors provides all directors with the conflict of interest policy, and all directors are required to sign individual copies of the policy annually and disclose any conflicts that arise on issues that are brought before the board and then abstain from participating in the discussion or voting on such issues. The Foundation has not employed an executive director since 2021.
