Brochures requesting donations were widely distributed in connection with the grand opening ceremony which took place in June 2018. Since the Organizations inception there has been an ongoing program of direct mailings and personal phone calls to an expanding list of professional and personal contacts as well as phone calls and mailings to other prospective and former donors. These activities are done by members of the board as well as individuals who donate their time for this purpose. 1.170A-9F 3 iii A PERCENTAGE OF FINANCIAL SUPPORT: During the first 4 years the Organization received several large donations which funded the land acquisition and a portion of the initial construction of the facility. The result is that the Organization received 15% of its support from public sources from 2015 to 2019. However for the year ended April 30 2019 the Organization received over 40% of its support from public sources. The fiscal year ended April 30 2019 is the year during which construction was completed and the Center began normal operations. This year is more representative of expected level of ongoing public support in future years. For the year ended April 30 2021 the Organization was forced to sharply curtail its fundraising effort for the entire fiscal year due to the Coronavirus pandemic. During the years ended April 30 2022-2025 the Organization received over 100% of its support from public sources. 1.170A-9F 3 ii B SOURCES OF SUPPORT: The Organization has received the majority of its support from individuals and businesses NOT related to the organization. In addition to direct donations several individuals and businesses have donated their time and services to assist the Organization in its exempt purpose. In the initial fundraising phase the Organization received several large donations as seed money. These donations paid for the acquisition of land and also paid for a portion of the facilitys initial construction cost. These large donations reduced the public support percentage. This is not expected to reoccur. Since the commencement of construction a significant portion of donations have consisted of smaller donations made by the general public. To date the Organization has received donations from over 100 unique donors. 1.170A-9F 3 ii C REPRESENTATIVE GOVERNING BODY: The Organization has a board of directors that governs its activities. Board members each have special knowledge or expertise to assist in fulfilling the Organizations exempt purpose including legal expertise financial expertise and experience with disabled persons. The board and its individual members do NOT represent the personal or private interests of a limited number of donors or related persons or businesses. Board members are bound to serve the public purpose of the Organization. In fact the bylaws provide for a formal written conflict of interest policy. No Director or Officer will have any financial interest in any entity which receives payment for services. Annually each board member must provide written confirmation of their compliance with this policy. The bylaws also provide that all board members and directors serve without compensation. The Organization has had no employees and has no plans to hire employees in the future. 1.170A-9F 3 ii D AVAILABILITY OF PUBLIC FACILITIES: The Organization was formed for the purpose of constructing and owning a day care facility for autistic mentally retarded and developmentally disabled adults. Construction of the facility was completed in June of 2018 at which time the facility began full time operations. The facility has operated on a full-time basis since that time. The facility serves disabled persons of the county where it is located. The entire facility is operated by Young Adult Institute Inc. YAI . YAI is an unrelated third party 501 c3 agency which is licensed by the New York State Office for People with Developmental Disabilities. YAI organizes and runs public programs and offers services to developmentally disabled individuals requiring various levels of support. Programs offered include healthy meal planning and food preparation cardiovascular health and weight management music and arts technology employment skills and opportunities for peer relationships and social interaction. Registration in these programs is open to the public. The programs are specifically designed for autistic mentally retarded and developmentally disabled adults and as such participation in the programs is limited to this group. This is the only limitation placed on participation in the programs. There is no cost to attend any of the programs at the facility; the programs are absolutely free for the participants. All of the participants are indigent and Medicaid eligible. YAI operates the facility pursuant to a 5 year lease and has done so continuously since the facility opened. The lease provides that YAI pays NO rent to the Organization for the use of the facility in delivering these programs. This was done to allow YAI to increase the amount of funds available to operate the public programs at the facility in order to serve more members of the community. YAI EIN # 11-2030172 is a public charity and receives 99% of their support from public sources. The facility is open every day during the week. The Board of Directors complete a Conflict of Interest Statement and Disclosure Questionnaire quarterly. Part VI, Line 19 The Conflict of Interest Policy and financial statements are available upon request during normal business hours. Part XII, Line 2c Due to the size of the Organization there are no committees with authority to act on behalf of the governing body.
