Shafer, CPA. A draft of Form 990 is reviewed by the CEO and Finance Director and then is presented to the Audit Committee for review and approval. The final version of Form 990 is provided electronically to all board members for review. Form 990 is discussed and approved at the next board meeting prior to being filed with the Internal Revenue Service and posted on agencys website. Pt VI, Line 12c The Sickle Cell Foundation of Palm Beach County annually provides a Conflict of Interest document to all staff and Board of Directors. Each are required to sign the statement. The statements are reviewed by the CEO and tracked by a staff member. It is the responsibility of the individual to make the Sickle Cell Foundation of Palm Beach County aware of any conflicts that arise after they sign the document. If there is a real or perceived conflict of interest an individual may participate in discussion around a given issue but will abstain from any vote pertaining to their conflict. Pt VI, Line 15a The compensation process for the CEO includes a review by the Executive Committee of the Sickle Cell Foundation of Palm Beach County and comparison to market.
