The Form 990 is reviewed at the board meeting by Cape Atlantic INKs CFO who has prepared the document. Once approved this approval is reflected in the minutes of the board meeting. Part VI, Line 12c All officers and directors and all full and part-time employees of Cape Atlantic INK are required to complete an Attestation Statement on a yearly basis. The Attestation Statement is supplied by the State of New Jersey Department of Children and Families DCF and is a requirement of our annual state contract. The Attestation Statements purpose is to disclose any conflicts or potential conflicts of interest. Any potential conflicts would be reviewed by Cape Atlantic INKs leadership and if applicable by the Board of Directors Executive Committee or full board if necessary in accordance with Cape Atlantic INKs Conflict of Interest policy. Part VI, Line 15 The Board of Directors of Cape Atlantic INK reviews all data collected relative to the evaluation of the agencys Executive Director. This includes the formal evaluation document prepared by all board members anonymously and an analysis of salary information prepared by the agencys CFO through the Guide Star Form 990 database. This analysis of salary information would normally include salaries of other Executive Directors in selected area non-profit agencies as well as comparable information of Executive Directors in other care management organizations CMOs. The full board reviews and approves any salary and or compensation increases for the Executive Director position this approval is documented in the agencys board minutes. The Operations Administrator CFO and all other management positions are reviewed by the Executive Director. Salary increases if any are made in accordance with the agencys yearly budget and any applicable state contract stipulations. Part VI, Line 19 Cape Atlantic INK has a policy on Public Inspection that is in compliance with IRS regulations. The agency will allow the public to access its Form 990 by in-person regular mail electronic mail and or facsimile fax requests as per its policy.
