Form 990, Part VI, Section B, Line 11b The board of directors is given a copy of Form 990 for review prior to filing with the IRS. Form 990, Part VI, Section B, Line 12c Board members & key employees must declare any conflicts of interest on a written Little Eden Conflict of Interest Statement and update their forms as changes occur. They also receive an annual reminder to update any changes to their forms. Form 990, Part VI, Section B, Line 15 The board of directors reviews and approves wages and benefits for the executive director and key employees on an annual basis. Form 990, Part VI, Section C, Line 19 The By-laws and a written Conflict of Interest Policy are both available to review upon request from the organization's office. The financial statements are reviewed annually at the annual members' meeting and are also mailed to any/all members on an annual basis, upon request.
