Form 990, Part VI, Section A, Line 7b Changes to our organization's governing documents must be voted on and approved by the general membership, as well as expenditures greater than 25% of our assets. Form 990, Part VI, Section B, Line 11b The Form 990 Online software system is used to create all the Form 990 documents. It includes cross-checking and validation of all responses before the forms are submitted. The Treasurer also reviews it to ensure it reflects the financial activity for the year. Form 990, Part VI, Section C, Line 19 The governing documents are available to the membership and have been filed with the state of Ohio. The financial reports are submitted to the Board of Directors and the membership. The form 990 documents are submitted to the IRS and similar forms are also submitted to the state of Ohio.
