We are open three days a week. This year, we served 13,548 households, or 58,555 people during the fiscal year and distributed over 989,867 lbs. of food. 18479 1835 Care House Clinic - a faith-based clinic providing basic medical, dental and mental health care for uninsured individuals in our community who live between 138%-250% of the federal poverty level. This year, we provided 77 medical, 174 counseling, 31 dental appointments and 307 visits to our Wednesday night vitals clinic. We provided $59,838 of medical and dental at no charge to our clients, and counseling services for a nominal fee. 5040 3842 Celebrate Recovery - A place where people battling emotional hang-ups and addictive behaviors can find support, teaching, and encouragement as they work their recovery process. We provide large group teaching sessions, open-share groups, and 12-Step Study groups. It is a Title 1 school of poverty to whom we provide supplies and encouragement gifts to the teachers. Part IX, Line 11g Other: Program Service Expense: we pay a monthly fee for a Medical Volunteer Provider Network so our clients who receive medical services at our clinic can access the services of medical specialists at no cost to the client. Part VI, Line 11b Copies of the Form 900 are sent to the Board of Directors and Finance Committee members via email for their review. Part VI, Line 12c Conflict of Interest Policy - A Conflict of Interest policy was approved during this current fiscal year and each member of our Board of Directors was asked to fill it out. This practice is now carried out at the initial meeting of each fiscal year. Part VI, Line 19 Each year, our financial documents are reported in our printed annual report booklet which is distributed to our board members, at our annual fundraising event, and at other public gatherings we participate in. When people ask to see these documents, we send digital copies of our governing documents, conflict of interest, and financial statements to them via email. Part VI, Line 3 Our human resources and bookkeeping tasks are facilitated by High Road PEO, under the supervision and direction of the COO/Programs Director and the Finance Committee. Part XI, Line 9 Building Project: This amount represents funds raised the previous year - 2023-2024 - that were in savings but were used this fiscal year to pay for expenses for our new food bank facility that do not fit into the functional expenses report nor our fixed assets report. Schedule B, Part 2 The donations we received regarding food from the Idaho Food Bank, and those related to donated rent and the payment of utilities from Nampa First Church of the Nazarene took place all year long, and not just on the February 28th date. The donations our Food Bank received from the Church of Jesus Christ, Latter Day Saints Bishops Orchards and Storehouse occurred on several occasions from August 1st, 2024 through October 15th, 2024. The donation by Curve Dental for our Care House Clinics dental service to utilize their dental records software is invoiced to us every month.
