We have an additional Summer Program that includes children through grade 3, who are siblings of currently enrolled Haven children. We are committed to providing safe, affordable, Christian daycare. We participate in the Illinois' state action for children program, and approximately 25% of our parents receive assistance from the state. We service Infants 6 weeks to Pre-Kindergarten age 6 during the school year and have a Summer Program for school-age children who are siblings of those currently enrolled so parents their children are in one site while they work. Our hours are 6:30 a.m.-6:00 pm Monday-Friday. We do not do a formal solicitation for students but use word of mouth and our website to advertise positions available in our classroom. Schedule E, Part I, Line 4 We do not offer scholarships and do not solicit contributions to cover our expenses, which are covered by the tuitions we collect from parents, grants written, and from occasional fundraisers for specific purchases. Form 990, Part VI, Section B, line 11b The President distributed the Form 990 to every member of the Board, who in turn reviewed the Form 990, and then voted to approve the Form 990 at its regularly scheduled Board meeting. After approval, the President then sent the return to the IRS. Form 990, Part VI, Section B, line 12c At our regularly scheduled meeting in June, prior to election of new and renewing Board of Directors, we address the issue of conflict of interest to ensure that no one on the Board has a known conflict of interest with Haven For Tots. Form 990, Part VI, Section B, line 15a The Haven Director and Business Manager (who has voice but not vote because she handles all phases of the business operation), and our Shopper position salaries are reviewed at our annual December budget meeting and approved prior to those salaries being paid to those Board members. Form 990, Part VI, Section C, line 18 Our Form 990 does not have to be filed with the State of Illinois; however, our Form 1023, Form 990, and all other documentation is kept in the office in a special binder, which is available by appointment for review in the office. Anyone who wishes to see our documentation can call and set up an appointment with our Business Manager to review these documents in our office.
