Chavarria received funds for a new prosthetic in the amount of $1,250. These funds of $2,500 were originally paid to Mr. Chavarria's doctor by the Houston Heights Lions Club and the club was reimbursed for half of the amount paid. The second grant was a payment to Ignis & Aqua Mechanical for repairs to Mrs. E. Hernandez' bathroom. Opportunities for Youth Contest 4 2600 0 4 students were awarded scholarships for winning our annual Opportunities for Youth essay contest. We had one first place winner for $1000, two second place winners for $600 each and one third place winner for $400. All scholarships were paid to the schools where the students were attending college. Students had to submit their information along with proof of enrollment. Sight Conservation 8 552 0 8 children were provided eye exams and glasses. The University of Houston Eye Institute sends the invoices directly to the District Bookkeeper. All checks are signed by two District officers and a copy of the check is kept with the invoice. Schedule I, Part I, Line 1 t Schedule I, Part I, Line 2 For Humanitarian Relief Grants, clubs must submit an application to the HRF Committee. The Committee then decides whether to award funds. The District contributes up to $1,250 for each approved grant request. The club making the request must also contribute matching funds equal to the amount being provided by the District. The Cabinet Bookkeeper prepares the check and keeps a copy along with the grant application and any receipts. For Sight Conservation, invoices are submitted for eye exams and glasses by the University of Houston Eye Institute to the District Bookkeeper. The bookkeeper prepares the check and it is signed by 2 Cabinet officers. A copy of the check is kept along with the invoice. Form 990, Part VI, Section A, Line 8b Cabinet meetings are held quarterly. The Cabinet Secretary is responsible for taking minutes at all meetings. Minutes are distributed to all Cabinet members and are also available to all District 2S2 Lions and the general public. Form 990, Part VI, Section B, Line 11b The District Cabinet (governing body) was given a copy of this report along with a copy of year end financials. The previous District Bookkeeper (who held the office for many years) was provided an advanced copy and reviewed this return, discussed any concerns, questions or recommended changes with the current bookkeeper. Form 990, Part VI, Section C, Line 19 The organization has placed all policies, governing and financial documents on its web page and encouraged all members to review them. The public can also review them. We filed an extension.
