SWAN is one of them.This program is supported with $86,290 in contributions from Monroe County found on the form 990, Page 9, Part VIII line 1(h). 158753 PRESERVATION: Under the NYS Division of Housing and Community Renewal Program we provide housing assistance to renters and homeowners, and community based organized activities in the areas of housing economics, community development and public safety.This program is supported with $220,220 in government grant and ESL grant contributions found on the form 990, Page 9, Part VIII line 1(h). 181920 98149 69048 OTHER PROGRAMS: The Settlement House Program provides additional support and services to our seniors, youth and families in the SouthWest Community. The Eviction Prevention Rental Assistance Program is administered by the County of Monroe and the City of Rochester. SWAN was awarded this grant to interview individuals, determine eligibility and administer rental arrears to property owners to maintain housing for Monroe County/City of Rochester residents facing eviction due to circumstances related to COVID.Emergency Services - Services provided include: food and clothing referrals, bus transportation, utility, rental and mortgage assistance, employment and job readiness referrals, food vouchers, summer lunch programs, holiday assistance, food, toys, and clothing, and holiday dinners. Income earned on these funds is unrestricted and used for general support of the Organization's exempt purpose. 1 Emergency Assistance 73 98149 Cost Rent assistance, clothing, furniture, repairs, etc. Part I, Line 2: Individuals who recieve grants are residents within the geographical area served. They must provide the Organization with proof of income, proof of household makeup, picture ID, and letter/notification/eviction notice/shut-off notice, etc. from utility service provider or landlord stating individual's name, account number, amount past due or needed to prevent service shut-off or eviction, as the case may be. For non-cash items, they must provide lease, statement or note from landlord stating apartment was unfurnished or proof that current appliance cannot be repaired. Individuals from outside the geographical area served are accepted if they are referred directly from another agency. Form 990, Part VI, Section A, line 6 SWAN has a one class of general members. Form 990, Part VI, Section A, line 7a Members may elect the Board of Directors. Form 990, Part VI, Section A, line 7b The bylaws of the Association may be adopted, altered, amended or repealed by a majority vote of the members of the Association. Form 990, Part VI, Section A, line 8b There are no other committees with authority to act on behalf of the governing body. Form 990, Part VI, Section B, line 11b A draft copy is reviewed by board members before it is filed. Form 990, Part VI, Section B, line 12c SWAN reviews the vendors used to ensure that there are no direct or indirect interests that could be perceived as a conflict of interest for the organization. If an appearance of a conflict of interest arises, it is first discussed with the Executive Director, Finance Manager, and Board President. If a conflict is determined to exist, SWAN will not proceed. Form 990, Part VI, Section B, line 15 The Board reviews compensation annually of the Executive Director. There are no other key employees. Form 990, Part VI, Section C, line 19 All volunteers receive a copy of our personnel policy manual, including a conflict of interest policy and governing policies at orientation. All funding providers and banks receive a copy of SWAN's financial statements. Form 990 is being filed late because the Organization has undergone a tremendous amount of turmoil in the past few years. In 2022 they had three Executive Directors. They also had their bookkeeper leave and they didn't have access to their detailed accounting records. They had to hire a new financial person to recreate the records. They are now on target, have the proper people in place and should be back to filing on time going forward.
