The evaluation includes a performance review, an evaluation of compensation for comparable positions, and feedback from other Board Members. A written substantiation of the review and compensation decision is then prepared. Form 990, Part VI, Section B, line 11b HPB's management team reviews the 990 and then distributes it to the Board of Directors before filing it with the IRS. Form 990, Part VI, Section B, line 12c The members of the Board of Directors must fill out or update a Conflict of Interest Survey annually. The HPB management reviews the completed surveys for conflicts. If there are conflicts, they are reported to the Chairman and President of the Board of the HPB Foundation.
