Because of the pandemic in 2020 most of our services were offered virtually or via telephone rather than in person. In 2021 we were able to provide more in-person services. About 35% of our services were provided in-person in 2021. Because it was needed this year we dedicated a significantly higher amount of funds to our Essential Needs program. Part VI, Section B, Line 11b A committee of three people two Board members and one staff member was authorized to compile the information necessary for the 990 and to complete the form with input and review by a volunteer CPA. The committee then review the form with the Board and Board members had a week to review the form and compiled information. Part VI, Section B, Line 12c Annually directors and key employees review the policy together and each completes and signs a certification and disclosure form. These forms are compiled and distributed to each person and are also available on our online database. All are responsible for monitoring proposed and ongoing transactions. The Board President and alternate are responsible for having the Board do a conflict of interest check prior to making decisions regarding transactions. This involves a checklist and a set of questions to determine if there is a conflict. We have a separate policy and procedure regarding conflicts after a transaction has occurred.
