This program provides daily meals and snacks. Shelter, eating utensils and paper products to serve food in a safe clean environment and any required assistive device needs. The patients get full course meal, a dessert and any special dietary request. All funds received are government food grant funds. One hundred percent of ADHAC is volunteer services and one key employee paid cooks daily meals for patients. No officers or volunteers are paid or reimbursed for any expenses they incur. All grant funds are used for meals and meal supplies, and meal preparation services. Schedule A, Part III, Line 12 ADHAC is one hundred percent government grant funded and volunteers. The only key employee is the cook who prepares meals for the patients. All receipts are kept for meals, meal preparation, and meal supplies such as paper products, plates, forks, spoons, Healthy meals are prepared daily for each patient in accordance with Department of Health and Human Services Food Grant compliance. Form 990, Part VI, Section B, Line 11b The officers and board members review semiannually the allocation of funds and financial statements to provide services to the patients of Adult Day Health Activity Center at board meetings. The grant funds are not enough to service the entire program, moreover the board votes to allocate funds to food and food preparation worker only. Monthly food purchases are documented and the program cook is paid by form W2. The W3 report and receipts to prepare form 990 are reviewed each quarter by officers and semiannually with the board. Review is done prior to filing the return with a hard copy form 990. Form 990, Part VI, Section B, Line 12c The Adult Day Health Activity Center does not reimburse any members or staff due to lack of funds. Our policy states no refunds to anyone. The program is funded by government grant funding and the program reduces funds each year because of lack of fund resources from the Department of Health and Human Services. All funds are needed for food and wages to the cook only. Form 990, Part VI, Section B, Line 15 Adult Day Health Activity Center (ADHAC) has only one employee who is the cook. ADHAC receives grant funds and uses those funds to purchase food and pay cook. The Cook was hired in 2021 and ADHAC follows the North Carolina Wage and Hour Act (NCWHA) that governs North Carolina employers' state wage and hour obligations NC General Statue Ann 95-25.1 to 95-25.25. Form 990, Part VI, Section C, Line 19 ADHAC provides all bylaws, policies, tax returns and supporting documents by request to any member of the public who request a copy. A hard copy or electronic copy can be provided if requested verbally, a email, or by a written letter request.
