Form 990, Part V, Line 2a: The Lighthouse for the Blind of Houston (LOH) employs on-site managers and maintenance staff who work for Lighthouse Living Centers No. 2, Inc. Living Centers No. 2 reimburses LOH for salaries, payroll taxes, health insurance and other payroll benefits. LOH files the required W-2s for these employees. Form 990, Part VI, Section A, line 3 The Lighthouse for the Blind of Houston (LOH), a related organization reported on Schedule R, provides management services for Lighthouse Living Centers No. 2, Inc. and receives a management fee of 5.5% of gross collections. Form 990, Part VI, Section A, line 7a The directors elected each year must be the same persons who are directors of the Houston Lighthouse Foundation. If any Foundation director is removed and/or replaced, then Living Centers No. 2 must remove/replace that director on its board as well. Form 990, Part VI, Section B, line 11b Form 990 is reviewed by the finance committee of the Lighthouse for the Blind of Houston.
