<Desc>Youth Court The Youth Court program represents a collaborative effort between Fontana School Police, Fontana Unified School District, Jurupa Unified School District, and Reach Out, serving as a voluntary diversion initiative that connects students with wraparound services designed to keep them out of the traditional juvenile justice system. This innovative program addresses first-time and minor offenses, ensuring all proceedings remain confidential. Youth Court has proven effective in preventing first-time juvenile offenders from progressing to more serious legal issues. This year, the program engaged 58 participants, demonstrating its ongoing success in promoting positive outcomes for youth in the community.Community Coalitions The Fontana Community Coalition (FCC) is a dedicated grassroots partnership that fosters positive opportunities for youth and celebrates Fontana's cultural diversity. FCC works to unite community members and organizations through media, policy initiatives, and capacity-building efforts to reduce and prevent substance misuse. Throughout the year, we participated in various events and launched the Partners for Innovative Communities (PIC) initiative in the City of Perris, empowering communities to create healthy and safe environments through policy design and collaborations. This year, we had a total of 53 participants across all coalitions, which include law enforcement, elected officials, youth organizations, and other community stakeholders. Additionally, the Morongo Basin Community Coalition (MBCC) focuses on reducing substance misuse in the Morongo Basin by building partnerships and facilitating discussions to promote a safer and healthier community.Opioid Awareness Training In our commitment to youth, we delivered Alcohol and Other Drugs (AOD) presentations to students aged 12-17 across the Inland Empire. These sessions help young participants understand the dangers of substance misuse and equip them with practical coping strategies. Notably, we reached all 7th and 8th grade students at South Ridge Middle School in Fontana, fostering a supportive environment for student wellness and advocacy. Our Opioid Overdose Response Training equips participants with vital knowledge about opioids, including how to recognize an overdose and administer Narcan. A life-saving medication. The training, offered in both English and Spanish, ensures participants leave with a Narcan Kit and the ability to act quickly in emergencies.Medication Assisted Treatment Program We are excited to announce the launch of the Medication Assisted Treatment (MAT) Program in the Morongo Basin. MAT is an evidence-based strategy designed to effectively treat opioid use disorder, combining FDA-approved medication with counseling and behavioral therapies to provide a holistic approach for individuals facing dependency challenges. Our community health workers have successfully supported community members, assisting with nine cases to date, and are dedicated to helping others on their path to recovery.National Innovative Communities ConferenceThe 14th annual National Innovative Communities Conference (NICC), Southern California's premier event focused on health equity and quality of life improvement, took place on June 11-12 at the Riverside Convention Center, drawing over 500 attendees, including community members, nonprofit leaders, healthcare professionals, policymakers, and business leaders. With the theme "Real Change Starts Here," the conference emphasized local action and bipartisan collaboration. Keynote speakers Dr. Yusef Salaam and Dr. Victor Rios shared impactful personal stories that underscored the importance of criminal justice reform and community-led change. The event also tackled the Inland Empire's health workforce shortage, featuring a Public Health Leadership roundtable and discussions on strategic solutions like creating an Academic Public Health Department and expanding paid internships. Attendees participated in 37 breakout sessions covering topics such as mental health and substance use prevention, and each received a Naloxone kit as part of the effort to combat the opioid crisis. NICC continues to be a vital platform for collaboration, shaping the future of the Inland Empire in anticipation of significant growth over the next decade.Healthy Jurupa Valley Healthy Jurupa Valley (HJV) creates an inclusive space for community members and partners to address local health priorities. Through regular meetings, workshops, and events led by five Action Teams, HJV transforms community concerns into concrete actions. Volunteer Action Team chairs, in collaboration with Reach Out, the City of Jurupa Valley, and other stakeholders, champion these initiatives, ensuring diverse voices are heard. Highlights this year included the 2nd Annual Da de los Muertos Celebration, 20 Little Free Libraries installed or maintained by Team CREATE, and literacy-focused events like the StoryWalk. The Community Prosperity Team hosted financial and skill-building workshops, while the Gardens & Markets Team promoted sustainability through events like Berries in Bloom and supported school garden projects. Additionally, the Safety and Readiness Team provided disaster preparedness resources and collaborated with Jurupa Youth United to foster traffic safety awareness. HJV remains a key force for community empowerment, education, and health in Jurupa Valley.Jurupa Youth UnitedJurupa Youth United (JYU), a youth-led community group under the guidance of Healthy Jurupa Valley's Safety and Readiness action team, is dedicated to creating a healthier and more equitable Jurupa Valley. Through impactful campaigns, community service, and resource events, JYU addresses key issues like youth involvement and substance use. This year, JYU hosted the Skate thru Summer event, a collaboration with Healthy Jurupa Valley, California Baptist University's Drug-Free Communities program, SCAG, and CalVIP. The event promoted a drug-free, safe, and healthy environment by offering safety education, distributing helmets and bike lights, and raising traffic safety awareness. Attendees enjoyed food trucks, resource booths, youth activities, giveaways, and a chance to win one of six new bikes.Health Equity Over the past year, the Health Equity team continued its mission to promote equity and inclusivity in addressing health disparities across San Bernardino and Riverside Counties. This was achieved by offering a diverse range of resources on topics like COVID-19, environmental health impacts such as heat awareness and water conservation, and assistance with the Individual Taxpayer Identification Number (ITIN). These efforts made significant strides in enhancing the wellbeing of the community. This year, the Health Equity team forged invaluable partnerships with the Salvadoran and Mexican consulates, expanding its reach to better serve the region. Through monthly presentations at both consulates, the team provided essential resources and health information to a broader audience. Additionally, the consulates each hosted dedicated days at our resource center, bringing their services directly to local community members. This proactive collaboration, alongside a strong social media presence, allowed the Health Equity team to effectively share reliable information on COVID-19 treatments and other crucial health resources with the community.Jurupa Valley Leadership Academy The Jurupa Valley Leadership Academy successfully graduated 16 community members this year, marking the completion of Cohort #5. This impactful 10-week program, which began on March 13, 2024, is designed to empower Jurupa Valley residents with the skills to connect fellow community members to essential resources and services. Participants engaged in skill-building, hands-on activities, and resource sharing across a range of topics, including leadership styles, effective communication, conflict resolution, immigration and advocacy, navigating the healthcare system, community safety, self-care, cultural competency, and trust-building. During their graduation on May 22, 2024, the cohort's 10 leaders presented on health topics of personal interest, showcasing their communication skills and deepening their commitment to community advocacy. These graduates continue to stay engaged with Reach Out, actively participating in community events and initiatives, demonstrating their dedication to making a positive impact in Jurupa Valley.Health Professions ConferenceThe annual Health Professions Conference (HPC), organized by Reach Out's Inland Health Professions Consortium, provided 388 high school students from across Inland Southern California with an in-depth look at healthcare careers on March 14, 2024, at UC Riverside. Students explored local healthcare workforce needs and career pathways through inspiring keynotes and hands-on workshops. Highlights included speeches</Desc>
