Patients must submit valid receipts for services covered before they are reimbursed. Part III, Line 3 3. Several services added in 2021 were discontinued in October 2023 due the to the high cost of providing the services. The foundation now provides reimbursement for medications, protheses, medical supplies and medical expenses. Part III, Line 4d Services included here were reimbursements to individuals for protheses ($3,108), medical supplies ($4,580), initial grant ($2,000), mortgage ($13,460), utilities ($10,327), auto fuel ($3,142), groceries ($11,175), transportation ($337), Massage ($259) and Pest Control ($241), the latter seven services were discontinued on October 1, 2023. Part VI, Line 11b Board members are provided a copy of this Form 990 and asked to review it and raise any questions they may have.
