The LAC-CAL's Board of Directors, who are not County employees, perform a ministerial function, and the management duties are performed by County employees. Form 990, Part VI, Section B, Line 11b The information entered in Form 990 is based on the LAC-CAL's annual financial statements which are audited by an independent accounting firm. Form 990 is completed, reviewed and signed by the employees of the County's Auditor-Controller. This form is completed by the LAC-CAL Accountant, a Principal Accountant, a Chief Accountant, and signed by the Division Chief. A copy of Form 990 is not provided to the Board of Directors before it is filed. However, the audited financial statements are provided to the Board for review and approval. Form 990, Part VI, Section B, Line 12c Each member must complete a Form 700 each year. This form is reviewed by the County on an annual basis. Form 990, Part VI, Section C, Line 19 LAC-CAL makes its governing documents, conflict of interest policy, and financial statements available to the public upon receipt of the public information request. Any member of the public may make such request to the County's Public Information Office who will direct the public to the appropriate office. The public may also submit their request directly to the LAC-CAL's administrative office. The documents are mailed upon request without charge, but postage is required for the larger documents. Records are made available for inspection, and for copying within ten (10) calendar days. In unusual circumstances, the ten (10) days may be extended by written notice from LAC-CAL's Administrator or from his or her designee, for no more than an additional fourteen (14) day period.
