Form 990, Part VI, Line 12c Conflict of interest policy Each Covered Person must promptly and fully report to UNEHEALTH's President situations that may create a conflict of interest when he or she becomes aware of such situations. In any situation where a Covered Person is in doubt, full disclosure should be made so as to permit an impartial and objective determination. A written record of the disclosure will be made. In the case where the President has a reporting obligation for a conflict of interest, he or she shall report to Vice President of UNEHEALTH. In addition to the ongoing disclosure obligation, UNEHEALTH shall annually send to all Covered Persons a copy of this Policy statement. The Covered Persons must complete an annual disclosure in COI Smart. The completed disclosure will be reviewed by the President or his or her designee(s). The President or the President's designee shall make such further investigation of conflicts of interest disclosures as he or she may deem appropriate.
