<DescriptionProgramSrvcAccomTxt>African Consortium for Quality Improvement Research (ACQUIRE) in Front line Health and Maternal Newborn Community Health Program (MNCH): The Africa Consortium for Quality Improvement Research in Frontline Health Care (ACQUIRE) is a not-for-profit organization based in Nairobi, Kenya. ACQUIRE is working towards empowering frontline healthcare workers and their institutions to apply continuous evidence-based quality improvement The ACQUIRE consortium is jointly led by Kijabe Hospital and Kenyatta National Hospital and takes a team-based, learning-by-research approach to Quality Improvement. Still in its nascent phase, ACQUIREs fiduciary operations are supported by the Africa Mission Healthcare (AMH) along with a Board of Directors that include representatives from the co-founding health care institutions. Equipping Africa has given startup funding to help this initiative grow (Total 90,000 ). In 2023 the fourth year of operations continued work on several significant engagements and continued work on a QI curriculum. 1. Quality Improvement Curriculum- ACQUIRE has begun convening experts to develop a curriculum in quality improvement that is context appropriate. This request came from Mbingo Hospital in Cameroon, where they wish to build a QI track into their Masters in Hospital Administration program. This will go live as a track in their Masters Program starting January 2024. 2. ACQUIRE together with the Kijabe College of Health Sciences completed the two QI collaboratives, one with faith-based hospitals who use mid-level providers (finished in May 2023) and another with engaging the Nyandarua County in addressing diabetic care finished in September 2023. 3. Successful continent wide story telling contest and launched a monthly webinar to build momentum for continuous quality improvement. Comments from the Graduating QI Teams This was very valuable. I hope these teams will be presenting at the next years Diabetes Society annual meeting -President of Kenya Diabetes Society I discovered we had resources that we were not using (monofilament) Team member It opened my eyes because measuring systematically showed that we were not doing as well as we had thought Physician at one of the hospitals This is the type of thing we need across the national system and seeing quality improvement teams is something we need to encourage Ministry of Health Return on Investment is that grew out of the seed money invested by Equipping Africa since 2020=6 month mentored course: ran with Stanford impacting 4 training hospital and 8 teams + 15 month mentored course for 3 teams IHI/ACQUIRE + leveraged work to attract $50,000 grant. The DEW Foundation funding sponsored a mentored continuous QI course bringing together leadership+ Driven a CQI 6 month mentored program to an entire County wide leadership for 7 teams in Nyandarua + Family Medicine Residents drove 3 teams of mid-level providers and this resulted in 3 posters on Research Day + a new $ 200,000 grant from an individual donor-Edna Walsh that so far has funded a continent wide QI storytelling contest and will directly train about 300 providers, has developed a training needs assessment for QI training needs that will gather data from institutions inside and outside Kenya and provide from a face to face conference in May to share QI evidence for action. MNCHP 15-year Celebration 2 Missionary trainings for embedded missionaries in high need areas Provided curriculum and mentorship for a 6-month long quality Improvement Course for an entire Countys health system working with 7 sets of teams from senior management to local dispensary. multiple new community health units-video highlights graduates in Tana River and Garissa areas. MNCH team members taught qualitative research methods to Kijabe School of Nursing, quality team working on hospital acquired infections, the second victim experience and Bethany Kids Childrens Center teams to better work with youth and families with Spina Bifida. Their support was essential to beginning to understand the how to as the actually lead focus groups and mentored new researchers in how to do this type of work.</DescriptionProgramSrvcAccomTxt>

<DescriptionProgramSrvcAccomTxt>Infectious Diseases: 1. Clinical infectious diseases Accomplishments The infectious disease fellowship at AKU was started with a substantial contribution from Equipping Africa and has continued to provide excellent training. The first graduate from our program is now the section head for infectious diseases and is known as a superb clinical and teacher. One of our two current fellows is just finishing his rotation at UBC and hopefully will be joining us as ID faculty in January. The UBC rotation was sponsored by Equipping Africa. The other fellow will be starting his 4-month UBC rotation in January and when that has finished, he will finish our program and return to his employment in the public system at Kenyatta National Hospital. Meanwhile, we have one of our AKU residency grads joining our fellowship in January and expect to have a Rwandese physician join our fellowship early this coming year. That is particularly exciting because it will be a new step in building ID capacity, not just within Kenya, but in other parts of East Africa. The University of Rwanda plans to start an ID fellowship so we will be helping with the beginning of that program. As we have previously emphasized, the four month rotation at the University of British Columbia (UBC) in Canada is helping to give them a well rounded training program with an international benchmark. This assures that they are learning and performing at international standards and to have experience illnesses that we see less frequently in Kenya. This is an important part of the program and Equipping Africa has been making that happen. 2. Antimicrobial stewardship Accomplishments Antimicrobial resistance is a growing concern globally and is finally starting to get appropriate attention. Some types of bacterial resistance is much greater here in Kenya than in the US, perhaps because there is even more widespread unnecessary use of antibiotics than in the US. It is a routine expectation of patients to be given antibiotics even for upper respiratory illness and because of the availability of antibiotics without prescriptions. Within the hospital, our antimicrobial stewardship program has focused on restricting certain antibiotics that are very broad spectrum in their activity. The ID team continues to be at the core of the activity and it is the ID fellows that make these activities possible. On the outpatient side, we have developed an e-learning course designed especially for medical officers and clinical officers. We initially gave the course to medical officers as well as pharmacy and lab technicians in the outpatient clinics of Aga Khan. We then offered the course to Penda Health, a set of clinics where clinical officers are the primary care providers (comparable to physician assistants or nurse practitioners). The uptake of the course there was much better than at AKU so we will have the chance to evaluate its impact. 3. Infection Prevention and Control Accomplishments As mentioned in the previous report, we offered the course entirely with in-country people. using our own Aga Khan-developed infection control practitioners, hospital quality, clinical microbiology and infectious disease specialists. Not only did it work, but was probably our best so far. We plan for that to become an annual event that will have the capability of reaching a greater number of hospitals and infection control personnel locally. For the 2023 course, Equipping Africa covered the tuition cost for some of the attendees while AKU Hospital covered the remainder of the cost. However, for this coming year, Equipping Africa will cover a much greater portion of the cost since the hospital does not have the budget to do it.</DescriptionProgramSrvcAccomTxt>
