The foundation also awarded scholarships to college students who are from our service area and are pursuing a degree in healthcare. Eligible applicants must have completed their freshman year of college and have been officially accepted into their program of study. The Sunrise Foundation accepted applications from students originally from Bowman County, Slope County, Harding County, and Western Adams County. Loan repayments are also offered to current employees of Southwest Healthcare Services who have been employed for at least 1 year. 500 500 The Foundation is organized to raise and invest funds to support the medical needs of the community of Bowman, North Dakota, and the surrounding rural service area. Part V, Line 4: The Travers endowment is used for the benefit of Southwest Healthcare Services. All other endowment funds are used for the benefit of Southwest Healthcare and any and all aspects of healthcare in our surrounding area. Part X, Line 2: The Foundation believes that it has appropriate support for any tax positions taken affecting its annual filing requirements, and as such, does not have any uncertain tax positions that are material to the financial statements. The Foundation would recognize future accrued interest and penalties related to unrecognized tax benefits and liabilities in income tax expense if such interest and penalties are incurred. 1 Scholarships 11 19375 0 Part I, Line 2: Grant requests are received twice a year from healthcare related entities. Invoices or quotes are provided with grant requests from Southwest Healthcare Services. The Board of Directors reviews the requests and makes the decision on awarding grants. All grant decisions are recorded in the board minutes. The Board of Directors sets an initial limit for scholarships. The pool of applications is looked at by the scholarship committee. The scholarship committee decides who qualifies for the scholarships and the amount they would like to award. They then take these recommendations to the Board of Directors for final approval. The Executive Committee shall have the authority of the Board of Trustees in management of the organization in the interval between meetings of the Board of Trustees. The Executive Committee is at all times subject to the control and direction of the Board of Trustees. Form 990, Part VI, Section B, line 11b The return is initially reviewed by Amanda Loughman, CFO of Southwest Healthcare, and Mariah Botha, Sunrise Foundation Director. A final review and approval process is made by the Board of Directors. Form 990, Part VI, Section B, line 12 Southwest Healthcare has a conflict of interest policy. Sunrise Foundation does not have their own conflict of interest policy, however they follow the same policies as Southwest Healthcare. Form 990, Part VI, Section B, line 15 The compensation of the Sunrise Foundation Director, Mariah Botha, was determined by the Evaluation Committee, which consists of the Chairman, Vice Chairman, Treasurer, and one of the Sunrise Foundation Directors, Becky Brooks. The recommendation to raise Mariah Botha's wages was then voted on by the Sunrise Foundation Board. A bonus was paid in 2024 that was based on 1.5% of total donations from January-December of 2023. The data from Results Plus was used to determine the total donations to base the bonus amount.
