The only requirement is that the employee has worked for the agency for a minimum of a year and worked a minimum 40 hours a week. Records are maintained in the employee's permanent employment file. Participants in the JHV EAP provide account numbers and paper documentation of their student loans in order to participate each year. Checks are made out and sent directly to the Loan Provider Processor Servicer accompanied by a letter of instruction to apply proceeds as elected by the employee. Employees do not receive any direct funding to insure funds are used as required. Part III Line 0 Column (b) All employees with a minimum one year of employment with JHV qualify,work a minimum 40 hours weekly, and can provide current documentation of having current student loans they are currently paying or are in forbearance for may apply to enroll in the program. Due to the complexity those with student loans in steep arrears or in collections do not qualify unfortunately. The desire is for the employees benefit from the program - not waste the funds the agency has to offer. Part VI, Section B, Line 1 The governing body did not delegate broad authority to any executive committee or similar committee - all board members share equal authority in decision making. Part VI, Line 8a Board members meet on as needed basis via mutual agreement via in person zoom email or phone call based on everyone's availability and the immediate needs of the agency but we try to meet at least one time a year in person to touch bases at minimum. Part VI, Line 11b Board members received a copy via email or mail at their preference to review said document and a paper copy is kept on premises available for their perusal at their request and also available to anyone in the local community who so desires to view said document. Part VI, Line 12c Members of the board are required to sign a compliance agreement at an annual board meeting held to discuss compliance and sign such document.
