Form 990, Part VI, Section B, Line 11b The Board Trustees are reviewing each section of the Form 990 for completeness and accuracy before filing the return Form 990, Part VI, Section C, Line 19 Governing documents disclosure explanation upon request. We have use the same accounting firm since our inception in 1994. We were under the assumption they were taking care of our return automatically just as they have done as in years past and were not aware that it was late until we received the notice from the IRS. Once we were made aware we compiled all the information and filed the return.
