Schedule E, Part I, Line 6 The Organization receives Financial Aid Scholarships through the Arizona Department of Education's Empowerment Scholarship Account (ESA). Form 990, Part VI, Section A, Line 2 Alex Trigonis and Alexandra Trigonis are husband and wife. Form 990, Part VI, Section A, Line 6 The organization has Members and no stockholders. Financial decisions or transactions in excess of $100,000 require approval from the Members, including any amendments to the governing documents of the Organization. Form 990, Part VI, Section A, Line 7a The Organization's Members have the power to elect or appoint Directors. Form 990, Part VI, Section A, Line 7b Financial decisions or transactions in excess of $100,000 require approval from the Members. Form 990, Part VI, Section B, Line 11b Form 990 is prepared by an accountant along with the financial statements for the Organization and reviewed by the Finance Committee. Form 990 is then distributed to each of the Directors Officers of the Organization for their review and approval. The Finance Committee then compiles any comments from the Directors and Officers and updates Form 990 accordingly. The updated Form 990 is then sent to the Organization's Treasurer for his final review, approval and signature. A copy of the completed Form 990 is kept by the Treasurer for the Organization's records. Form 990, Part VI, Section B, Line 15 Annual salaries are reviewed and approved by the Board of Directors at the Annual Meeting as part of the Annual Budget Review. Any bonuses or other compensation recommended by the Organization's Principal or any Officer or Director are also reviewed and approved by the Board of Directors. The Finance Committee also conducts annual research of the compensation for Principals and teachers to present to the Board of Directors at the annual meeting. Form 990, Part VI, Section C, Line 19 The Articles of Incorporation for the Organization, including the known place of business, Statutory Agent's address and contact information are publicly available through the Arizona Corporation Commission's website at azcc.gov.
