Each of these lists is reviewed monthly to ensure current program participation and eligibility before stipends are dispersed. Additionally, youth participants may receive material assistance in the form of purchased clothing, technology, food, or other timely needs as assessed by case management personnel. Detailed records of all assistance provided are kept within participants case management files, as well as being tracked within organizational accounting software and reported to funders as necessary. Form 990 governing body review Part VI line 11 The form was prepared and reviewed by multiple directors prior to filing including the Treasurer and the Co-Director of Operations. Conflict of interest policy compliance Part VI line 12c Directors, employees, and key volunteers are required to review and sign the Conflict of Interest policy annually. Individuals are expected to disclose any time when they may have an actual or perceived conflict of interest, and are also welcome to raise concerns about the potential conflicts of other individuals. There is then group discernment about whether a conflict exists and, if so, how it must be taken into account in any applicable decision making. Governing documents etc available to public Part VI line 19 All of the organizations governing documents, conflict of interest policy, and financial statements are available to the public upon request. Some of these documents are also publicly available on the Candid/Guidestar website.
