Specifically, the Board resolution amended the organization's Articles of Incorporation to remove the word "Cooperative" from its name, per New Mexico Secretary of State reguluations prohibiting the word "Cooperative" in the legal business name of any domestic non-profit organization. The Board resolution changed the name of the entity to "Hastaadi Neeznadiin DBA Gallup Health", accordingly, to reflect the community it serves, reflected in the amended Articles of Incorporation. Electronic filing completed within ten days upon receipt of IRS Letter 2695C, which requires tax-exempt organizations to file electronically. The 990-EZ form and required Schedules were remitted to the IRS via US Mail due to a misunderstanding of surrounding Section B of 990-EZ, in which the entity underwent a Name Change, in which it was mistakenly thought to be required to remit a paper return with supporting legal documentation of said Name Change.
