Part VI, Line 11b Form 990 was presented at the annual board meeting June 2022 Part VI, Line 19 All forms 990 and all governing, documents, conflict of interest policy, financial statements, and federal tax return are available to the public upon request from our office located at 401 1st Ave SW Ardmore OK 73401 During this time we were required to move out of the Goddard Center that housed all of our documents on the second floor. We were unable to meet due to State COVID Restrictions. There is no elevator to the second floor offices where the files are kept that have the documentation in regards to our submission. I am in a wheelchair due to COVID and can't climb the stairs or enter the building to even find where the documents have been pushed to the side. We are still not allowed in the building due to Asbestos Abatement and then Construction which is still ongoing on as of this date. It was supposed to be completed last year but we still are displaced. Based on the one copy I have of what was submitted I am filling out this form.
