They person who oversaw taking care of things such as this had very recently passed away. We worked diligently to ensure that all of the needed information was at hand. When putting the proper information into the system, I was presented a screen requesting information that was not included in the files of which I was in possession. I was not going to be able to locate the needed information with enough time and still make the deadline. I requested an extension- which was granted. I have worked to make sure that such an occurrence will not arise going forward.
